There are evidence indicating that endoscopic retrograde cholangiopancreatography with sphincterotomy (ERCP+S) reduces the rate of complications in patients with severe acute biliary pancreatitis (ABP). The aim of this study was to analyze our experience in the treatment of severe ABP with ERCP+S and compare the evolution of the patients treated early (3 days or less following admission) with those treated late (after the 3rd day). A retrospective review of the clinical histories of the 16 patients with severe ABP treated with ERCP+S over the last two years is presented, analyzing their clinical features on admission and evolution. Biliary stones or biliary sludge was observed in 11 cases (69%). Nine patients had undergone early, and 7 late, ERCP+S. The patients of the latter group presented a greater number of complications during evolution than the former group (43% vs 0%; p = 0.025). The mortality of the patients of the late ERCP+S was also greater although without statistical significance (28.6% vs 0%; p = 0.15). No complication secondary to ERCP+S was detected. These findings confirm the fact that ERCP+S performed early in severe ABP significantly reduces the number of complications and is associated with a lower mortality.  相似文献

This study explores the use of Markov models in some areas of systems analysis in which time evolution of the system may be a significant factor in influencing the system reliability or availability. Comparisons are made between the Markov models and the time-averaged fault tree models for determining support system failure initiating event frequency in a nuclear power plant, for both power and shutdown conditions. Factors affecting consistency between the fault tree approach and the Markov model approach are studied for systems with common two train configurations. A correlation is developed to estimate the ratio between initiator frequencies through both approaches for a two parallel component system. Insights are developed as to when time averaged and simplified fault tree models support a good approximation to the more rigorous time-dependent Markov models.  相似文献

The removal of nickel ions from dilute solutions using a process that combines an ion-exchange bed with electrodialysis has been studied. The main aspects include: the concentration of nickel ions in the diluate, the voltage over the cell and the current density distribution along the ion-exchange bed. The current density distribution provides insight into the state of the bed as it is simultaneously loaded with Ni2+ and regenerated with an electric potential difference applied perpendicular to it. A simple model is used to describe the state of the bed and the quantity of nickel removed from it as a function of time. Under specific conditions the precipitation of metal hydroxides is observed in the compartment containing the ion-exchange bed. The results show that hydroxide precipitation is related to the nickel concentration in solution and the electric potential gradient across the bed.  相似文献

A case study was carried out to investigate the bed agglomeration observed in a fluidized bed incinerator when burning blends of three wastes (carbon soot, biosludge and fuel oil). Several instrumental approaches were employed (i.e. XRF, SEM, XRD, and ICP-AES) to identify the bed materials (fresh sand and degrader sand) and clinkers formed in the full-scale incinerator tests. Several elements (V, Al, S, Na, Fe, Ni, P, and Cl), which normally are associated with the formation of low melting point compounds, were found in the waste blends at high content levels. The clinker bridges were identified to be associated with Al, Fe, V, K, Na, S, Ni, and Si elements.The effects of temperature and blending ratio were investigated in a muffle furnace. Carbon soot is believed to be more susceptible to the clinker formation than the other two fuels. Thermodynamic multi-phase multi-component equilibrium calculations predict that the main low melting point species could be Al2(SO4)3, Fe2(SO4)3, Na2SO4, NaCl, Na2SiO3 and V2O5. This information is useful to understand the chemistry of clinker formation. Also, it helps to develop methods for the control and possible elimination of the agglomeration problem for the design fuels.  相似文献

In this paper, coupled dynamics are presented for two cooperating mobile robotic manipulators manipulating an object with relative motion in the presence of uncertainties and external disturbances. Centralized robust adaptive controls are introduced to guarantee the motion, and force trajectories of the constrained object converge to the desired manifolds with prescribed performance. The stability of the closed-loop system and the boundedness of tracking errors are proved using Lyapunov stability synthesis. The tracking of the constraint trajectory/force up to an ultimately bounded error is achieved. The proposed adaptive controls are robust against relative motion disturbances and parametric uncertainties and are validated by simulation studies.  相似文献

Experiments on white rats proved that the silicotic pulmonary changes slowed with the tails periodically immersed into iodine solution. Those pulmonary changes were due to inhalation of quartzite dust and especially due to intratracheal administration of standardized quartz dust DQ12. Influenced by inhalation of dust containing quartz, the rats demonstrated characteristic alterations in cytologic features of bronchial and alveolar lavage water. The "iodine baths" also declined those alterations, that may prove higher resistance of pulmonary macrophages to cytotoxic effects of quartz. This hypothesis finds a support in experiments with culture of rat peritoneal macrophages. If incubated in a medium with low iodine concentrations and subsequently washed off, the macrophages demonstrate lower incidence of cytotoxic effects due to the next incubation with particles of DQ12. The macrophage's higher resistance against cytotoxic agents is believed to result directly in antisilicotic effect of iodine, so "iodine baths" could be recommended for pathogenetic therapy and medical prophylaxis of silicosis.  相似文献

A representation of local image structure is proposed which takes into account both the image's spatial structure at a given location, as well as its deep structure, that is, its local behaviour as a function of scale or resolution (scale-space). This is of interest for several low-level image tasks. The proposed basis of scale-space, for example, enables a precise local study of interactions of neighbouring image intensities in the course of the blurring process. It also provides an extrapolation scheme for local image data, obtained at a given spatial location and resolution, to a finite scale-space neighbourhood. This is especially useful for the determination of sampling rates and for interpolation algorithms in a multilocal context. Another, particularly straightforward application is image enhancement or deblurring, which is an instance of data extrapolation in the high-resolution direction.A potentially interesting feature of the proposed local image parametrisation is that it captures a trade-off between spatial and scale extrapolations from a given interior point that do not exceed a given tolerance. This (rade-off suggests the possibility of a fairly coarse scale sampling at the expense of a dense spatial sampling large relative spatial overlap of scale-space kernels).The central concept developed in this paper is an equivalence class called the multiscale local jet, which is a hierarchical, local characterisation of the image in a full scale-space neighbourhood. For this local jet, a basis of fundamental polynomials is constructed that captures the scale-space paradigm at the local level up to any given order.  相似文献
