Outcome of different treatment mixes in a multidimensional neuropsychological rehabilitation program.

Within a clinical demonstration program, 59 traumatically brain-injured patients were treated with 1 of 3 mixes of treatment. Mix 1 included cognitive remediation (CGR), small-group interpersonal (SGI) communication training, therapeutic community activities, and personal counseling. Mix 2 was similar to Mix 1 but stressed SGI exercises and eliminated CGR. Mix 3 emphasized CGR and eliminated SGI exercises. The efficacy of the treatment mixes was evaluated with performance on neuropsychological tests, improved independence in functional activities, measures of intra- and interpersonal functioning, and vocational outcome. Ss' participation in the program, irrespective of treatment mix, yielded improvements in (1) self-image, (2) quality of interpersonal relatedness and interaction, (3) involvement with others in naturalistic settings, and (4) vocational outcome. Limited reinforced space in segmental retaining walls

Current design of geosynthetic reinforced segmental retaining walls considers an a priori limitless length for reinforcement installation. Such length is typically 0.5–0.7 times the height of the wall. However, often there are constraints on such space; e.g., bedrock formation located at a small distance behind the facing. The objective of this note is to introduce a procedure for assessing the required long-term strength of the reinforcement while considering its limited length. Predictions by a conventional slope stability analysis were first checked against a continuum-mechanics based numerical analysis. Upon obtaining good agreement, a design chart was developed. The chart enables the determination of the reduction in the lateral earth pressure coefficient due to the constrained space. The revised earth pressure coefficient can be used with current analytical methods to account for the limited space. The results appear to be valid for conventional walls retaining a limited volume of soil. Comparison with limited experimental results for unreinforced backfill shows reasonably good agreement.     

A Comprehensive Assessment of Qualitative and Quantitative Prodromal Parkinsonian Features in Carriers of Gaucher Disease—Identifying Those at the Greatest Risk

Carriers of GBA1 gene variants have a significant risk of developing Parkinson’s disease (PD). A cohort study of GBA carriers between 40–75 years of age was initiated to study the presence of prodromal PD features. Participants underwent non-invasive tests to assess different domains of PD. Ninety-eight unrelated GBA carriers were enrolled (43 males) at a median age (range) of 51 (40–74) years; 71 carried the N370S variant (c.1226A > G) and 25 had a positive family history of PD. The Montreal Cognitive Assessment (MoCA) was the most frequently abnormal (23.7%, 95% CI 15.7–33.4%), followed by the ultrasound hyperechogenicity (22%, 95% CI 14–32%), Unified Parkinson’s Disease Rating Scale part III (UPDRS-III) (17.2%, 95% CI 10.2–26.4%), smell assessment (12.4%, 95% CI 6.6–20.6%) and abnormalities in sleep questionnaires (11%, 95% CI 5.7–19.4%). Significant correlations were found between tests from different domains. To define the risk for PD, we assessed the bottom 10th percentile of each prodromal test, defining this level as “abnormal”. Then we calculated the percentage of “abnormal” tests for each subject; the median (range) was 4.55 (0–43.5%). Twenty-two subjects had more than 15% “abnormal” tests. The limitations of the study included ascertainment bias of individuals with GBA-related PD in relatives, some incomplete data due to technical issues, and a lack of well-characterized normal value ranges in some tests. We plan to enroll additional participants and conduct longitudinal follow-up assessments to build a model for identifying individuals at risk for PD and investigate interventions aiming to delay the onset or perhaps to prevent full-blown PD.     

Mechanically stabilized walls over soft clay: Geosynthetic to prevent deep-seated failures

Mechanically stabilized walls can sometimes be economically constructed over poor foundations such as a soft clay. A major design concern in such a case, however, is the stability against deep-seated failure where the potential slip surface passes through the unreinforced backfill and the clayey foundation. To attain a prescribed safety margin against deep-seated instability, a high-strength geosynthetic sheet, placed at the backfill-clay interface, can be used. The objective of this paper is to present an analysis method which enables the user to determine the required distribution of tensile resistance of the reinforcing sheet and therefore can be used as a design tool. The analysis is based on limit-equilibrium and by virtue of the mechanism used it is rigorous in the sense that no statical assumptions are utilized. Its predictions compare favorably with some conventional methods; its application, however, is easier. It is demonstrated how the analysis results can be condensed into a useful format of design charts. Since the direction of the reinforcement's tensile resistance is not known, a zone is suggested within which is the probably required distribution of this force.     

Personal resources and political uprooting: The Israeli experience.

The authors assessed distress in Israeli society before and after Israel’s disengagement from Gaza and the northern West Bank during the summer of 2005. Random samples of 701 Israelis residing in various parts of Israel (Study 1) and 246 Israelis residing in Gaza and the northern West Bank who were to be uprooted (Study 2) were interviewed by telephone several weeks before the disengagement (T1), with 462 and 124 of these respondents, respectively, reinterviewed several weeks after the disengagement (T2). Anxiety related to the national and security situation was lower at T2 than at T1 in both studies. A personal resource, namely mastery–optimism, contributed negatively to both anxiety and hostility before the disengagement in both studies. In Study 1, it also predicted lower postdisengagement anxiety. The findings underscore the importance of mastery–optimism in the adjustment to threatening political events such as forced uprooting. Use of cellular confinement for improved railway performance on soft subgrades

Due to extensive right-of-way, railroads are inevitably subject to poor subgrade conditions and interrupted service for significant maintenance due to excessive deformations and loss of track geometry. Geocell confinement presents itself as a possible solution for improving performance of ballasted railroad embankments over weak subgrade. To investigate the efficacy of geocell confinement on ballasted railway embankments, a set of well-instrumented, large-scale cyclic plate loading tests and numerical simulations were performed on geocell-confined ballast overlaying a weak subgrade material. The agreement of results from tests and simulations served as a basis for simulating practical track geometry and performance for various geocell configurations and subgrades using three-dimensional (3D) finite element (FE) analyses. The study showed that geocell reinforcement significantly decreased track settlement, decreased subgrade deformations with lower and uniform distribution of vertical stresses on subgrade and inhibited lateral deformation and serviceability under cyclic loading. These results demonstrate that geocell confinement can be an effective alternative to subsurface improvement or shorter maintenance cycles, particularly on weak subgrades.     

Determination of trace isoflavone phytoestrogens in biological materials by capillary electrochromatography

Capillary electrochromatography using a specialty monolithic matrix was utilized in developing a rapid and highly efficient separation of isoflavones in biological materials. Without a preconcentration technique, it is relatively easy to reach ppm-ppb concentrations of these compounds in soy-based foods and verify them structurally using a photodiode array detector. With on-column preconcentration, we were able to measure low-ppb levels in human serum. Using blood samples from human volunteers, whose diet was supplemented by a soy-based product, the method has been validated for high-throughput screening of isoflavones in clinical studies.

Let X and Y be two run-length encoded strings, of encoded lengths k and l, respectively. We present a simple O(|X|l+|Y|k) time algorithm that computes their edit distance.     

Clinical assessment of attachment patterns and personality disorder in adolescents and adults.

The relevance of attachment theory and research for practice has become increasingly clear. The authors describe a series of studies with 3 aims: (a) to validate measures of attachment for use by clinicians with adolescents and adults, (b) to examine the relation between attachment and personality pathology, and (c) to ascertain whether factor analysis can recover dimensions of attachment reflecting both interpersonal and narrative style. In 3 studies, experienced clinicians provided psychometric data using 1 of 4 attachment questionnaires (2 adolescent and 2 adult samples). Attachment dimensions predicted both personality pathology and developmental experiences in predictable ways. Factor analysis identified 4 dimensions that replicated across adolescent and adult samples on the basis of a combination of interpersonal and narrative indicators: secure, dismissing, preoccupied, and incoherent/disorganized.
