Trauma is the major source of mortality in the pediatric population. A retrospective review was performed on patients admitted to the Children's Hospital and Health Center Trauma Program, San Diego, California, from August 1984 to May 1990. The purpose of this review was to evaluate pediatric trauma and to determine the best treatment and evaluation for genitourinary injuries. Blunt trauma was responsible for 98 percent of the injuries, with renal injuries being the most common. Bladder (7) and male urethral (2) injuries, and vaginal lacerations (8) also occurred. The most severe renal injuries (70%) and all significant bladder and urethral injuries were associated with gross hematuria. Hypotension was present in 31 percent of patients but rarely required surgical exploration for correction. Eighty-six patients underwent radiographic imaging. Computerized tomography (CT) scans demonstrated the most information about intra-abdominal solid organ injuries but was inaccurate in detecting bladder or urethral injuries. Genitourinary injury is common in children but rarely requires surgical management. CT scan is the best study to determine extent of solid-organ injury but is inferior to cystourethrography to diagnose bladder or urethral injuries.  相似文献

Bladder epithelium relies primarily on the presence of a surface glycosaminoglycan (GAG) layer and the structural integrity of cell-cell contact to maintain impermeability to toxic urinary wastes. Previous clinical studies evaluating bladder permeability characteristics in interstitial cystitis patients had indicated that epithelial desquamation occurs after treatment with protamine sulfate (PS) followed by hypertonic urea. The following study was performed using rabbits to further investigate this finding. The urinary bladder was evaluated for optimal treatment conditions for epithelial removal. Protamine sulfate (1 to 10 mg./ml.) and urea (100 to 200 gm./ml.) were instilled into the bladder at volumes ranging from 5 to 60 ml. to that required for near maximum distention. After incubation at room temperature for 15 minutes, the bladders were fixed and evaluated histologically for epithelial removal. The maximum epithelial removal occurred when the bladders were distended, and when PS concentration was 5 to 10 mg./ml. and urea at 200 gm./l. There was greater epithelium removal after repeated treatments. Epithelial cells that were removed were not viable based on Trypan blue staining. There was no significant increase of C14 labeled urea in the plasma after 15 minutes. Rabbits that were followed for 6 weeks after treatment did not show any histological evidence of increased collagen deposition and/or fibrosis. This procedure may have important clinical value since it may remove sufficient bladder epithelium in patients with transitional cell carcinoma to have therapeutic benefit. This offers a realistic option for selective, nontoxic destruction of bladder epithelium.  相似文献

10 patients were subjected to tandem transplantation for breast cancer (n = 3), ovarian cancer (n = 2) and multiple myeloma (n = 5), at the Second Department of Medicine, Donauspital, Vienna. The breast cancer patients were in stages 2 and 3, respectively, at diagnosis and entered complete remission thereafter. 2 of them developed lymph node metastasis and additional local recurrence, the 3rd patient presented with distant metastasis. The 2 patients with ovarian cancer were in stages Figo III and IV, respectively, at the time of diagnosis, and showed minimal residual disease at second-look-operation. 5 patients with multiple myeloma were in stage 3 pretransplant. Peripheral stem cells were obtained after either high-dose cyclophosphamide or FEC induction and application of cytokines. In 4 patients, tandem transplantation has been completed. 1 patient with multiple myeloma, who had received total body irradiation in combination with chemotherapy for the 2nd transplant, succumbed from idiopathic interstitial pneumonia. No severe clinical complications were observed in all other patients. All patients with solid tumors entered complete remission after the 1st transplantation. 3 of them completed tandem transplantation. Of these, 2 remain in continuous complete remission, the 3rd patient relapsed in lymph nodes day 485. In patients who received only 1 course of high dose chemotherapy with stem cell transplantation, relapses occurred on days 29 and 75, respectively. All patients with multiple myeloma entered only partial remission. We conclude that supralethal chemotherapy with peripheral blood stem cell support is a safe procedure that may at least induce prolonged remissions in solid tumors and hematologic malignancies.(ABSTRACT TRUNCATED AT 250 WORDS)  相似文献

A field model code, JASMINE, has been adopted to calculate ceiling jet flows caused by plumes from unsteady fire sources in a large corridor. The idealized fast- and medium-fire energy release rates, obeying thet 2 law for fire growth, were used. The effects of vents were studied by simulating three different configurations: a centrally located vent—that is, a vent directly above the fire source; an eccentrically located vent; and no vent. The results were compared with recent large-scale experiments conducted at the Swedish National Testing and Research Institute (SP) and with results from the computer program LAVENT. Some calculated sprinkler temperatures with three different RTI values are also presented.The present JASMINE simulations agree reasonably well with measured ceiling temperatures and velocities in large-scale tests. In general, the calculated ceiling jet temperatures are slightly overestimated and the velocities slightly underestimated. Also, the layer thicknesses calculated by JASMINE are somewhat thinner than those measured. The ceiling jet theory and present LAVENT results predict very thin layer thicknesses. However, these theories are only valid when wall effects are eliminated. From the present study it can be concluded that CFD models are generally more accurate when used to predict confined and unconfined flows.  相似文献

A laboratory test facility for solar radiation detectors has been built and is in operation at the Aerological Station of the Swiss Meteorological Institute (SAP/SMI). This installation is conceived as a universal test bed for solar radiation exposed meteorological instruments, and consists of a commercially available solar simulator, a laser alignment system, a translation mechanism with instrument mounts, and an adjustable projection mirror. The solar simulator produces a well characterized radiation beam which can be filtered to match the terrestrial or outer space solar spectrum with an irradiance of up to one solar constant (1367 Wm−2). The instrument mounts and a HeNe laser beam provide a precise and easy alignment of the reference and the test instruments in the radiation beam, allowing for incident angles in the range of 15°–75°. The measurement is based on a comparison of the response of an active cavity absolute radiometer PMO6 with the signal of the test instrument.Detailed investigations of the Haenni Solar 111B type heliometer have revealed important irregularities in the sunshine threshold irradiance angular distribution. Measurements performed with and without the protection glass cover prove the exceedingly high threshold values at large declination angles to be a consequence of enhanced reflections due to the incident angle and inhomogeneities in the glass cover.Very satisfactory results have also been obtained on characterization measurements of pyranometers showing the mean values of the responsivity to be within 0.8% of the calibration values measured at the world radiation center (WRC) at Davos.  相似文献
