An autopsy case of cytokeratin 7-positive minute adenocarcinoma of the lung with systemic metastases

We describe a 60-year-old woman with leg pain. Although metastatic bone tumor and atypical cells mimicking signet-ring cells in the bone marrow picture were observed, systemic survey revealed no primary lesion. The patient died two months after admission from systemic progress of the disease. Autopsy revealed a small focus of adenocarcinoma within the right upper lobe of the lung and systemic metastases without any particular changes in the gastrointestinal tract. The tumor cells of the lung were diffusely positive for cytokeratin 7, whereas cytokeratin 20 immunoreactivity was weak and focal, and that supported the lung origin of the present tumor. Moreover, the tumor cells in the bone marrow showed a similar pattern in immunoreactivity. These findings suggest that cytokeratin 7 and cytokeratin 20 immunoreactivity is helpful for the premortem diagnosis of the metastatic tumor of unknown origin.     

Sarcoidosis associated with acute renal failure and increased levels of interleukin-6 in urine

OBJECTIVE: To evaluate the incidence of non-specific granulomatous prostatitis (GP) in our series of prostate biopsies and to verify whether there were differences in the features of DRE, PSA and ultrasound findings in patients with GP and patients with prostate cancer that could be used as clinical indications in GP diagnosis. MATERIAL AND METHODS: Between 1994 and 1996, 835 patients with prostatic syndromes underwent echoguided transrectal biopsy. Neoplasia was diagnosed in 323 (39%) patients, non-specific GP in 11 (1.5%), whereas no malignancy signs were found in the remaining 501 (59.7%). A retrospective comparison of DRE features, PSA levels and the existence of echographic nodes was conducted between cancer patients and GP patients. RESULTS: 55% GP patients had suspicious DRE; in 64% at least one node with different echogeneicity was identified in the transrectal ultrasound, and the mean PSA value was 17.3 ng/ml. When they were compared to the group of patients with prostate cancer, no significant differences were found. CONCLUSION: In our experience we have not found any specific feature in DRE. PSA levels or ultrasound examination that allows to differentiate GP from prostate cancer. Transrectal biopsy of the gland is essential for the differential diagnosis of both entities.     

A surgical case for the huge chronic expanding hematoma developed in the thoracic cavity

We report a case of operation against chronic expanding hematoma developed in the pleural cavity of the patient who were treated as anaplastic anemia. 69-year-old man had been underwent thymectomy because of the thymoma and hemogenic pleural effusion 8 years ago. Then, last year, he was pointed out severe thrombocytopenia and underwent transfusion of thrombocyte. In the same period, chest X-ray examination revealed development of the tumor-like shadow in the left lung field. The abnormal shadow has enlarged, so the patient admitted us. X-ray findings revealed the huge tumor occupied completely the left pleural space. He was also pointed out severe pancytopenia (WBC: 2,600/mm3, RBC: 172 x 10(4)/mm3, thrombocyto: 1.9 x 10(4)/mm#), and the examination of bone marrow revealed anaplastic anemia. Thoracotomy was underwent, and the tumor was resected. Pathological diagnosis of the tumor was huge hematoma. Negative pressure in the pleural space would keep the hemorrhage and make hematoma huge. The hematoma in the pleural space should be resected as soon as the diagnosis is decided.     

Water-filled single-wall carbon nanotubes as molecular nanovalves

It is known that at low temperature, water inside single-wall carbon nanotubes (water-SWNTs) undergoes a structural transition to form tube-like solid structures. The resulting ice NTs are hollow cylinders with diameters comparable to those of typical gas molecules. Hence, the gas-adsorption properties of ice- and water-SWNTs are of interest. Here, we carry out the first systematic investigation into the stability of water-SWNTs in various gas atmospheres below 0.1 MPa by means of electrical resistance, X-ray diffraction, NMR measurements and molecular dynamics calculations. It is found that the resistivity of water-SWNTs exhibits a significant increase in gas atmospheres below a critical temperature Tc, at which a particular type of atmospheric gas molecule enters the SWNTs in an on-off fashion. On the basis of this phenomenon, it is proposed that water-SWNTs can be used to fabricate a new type of molecular nanovalve.     

Effect of alternating combination chemotherapy consisting of cyclophosphamide, doxorubicin, vincristine, cisplatin, and etoposide for small cell lung cancer on hematopoietic progenitors in the peripheral blood

The effects of a combination chemotherapy (CAV-PVP) consisting of cyclophosphamide, doxorubicin, hydrochloride (dox) and vincristine (CAV) alternating with cisplatin and etoposide (PVP) on peripheral blood hematopoietic progenitor cells (PBHPs) were studied in five patients with small cell lung cancer (SCLC). The kinetics of the CFU-GM levels were different during the CAV and PVP phases. None of the five patients displayed a rebound increase in the level of peripheral blood CFU-GM during the CAV phase. In contrast, all five patients displayed a rebound increase in peripheral blood CFU-GM levels during the PVP phase of the alternative combination chemotherapy (3-5 weeks after the initiation of PVP regimen). These findings indicate the optimal timing for leukapheresis to obtain PBHPs in SCLC patients which have been treated with an alternating combination chemotherapy consisting of CAV-PVP.     

Second generation hepatitis C virus antibody-positive rate in children: investigation of the route of hepatitis C virus infection in children with no history of transfusion

BACKGROUND: Proponents of laparoscopic colectomy claim a more rapid return of gastrointestinal function, but objective data supporting this contention are lacking. Our aim was to compare the duration of delay in gastrointestinal transit after total laparoscopic, laparoscopic-assisted, and open colonic resection. METHODS: Thirty-nine female dogs were assigned to three groups: group 1, laparoscopic colectomy; group 2, laparoscopic-assisted colectomy; and group 3, open colectomy. Transit studies were performed before operation and on days 1, 2, 3, and 5 after a mid-descending colectomy with end-to-end anastomosis. Gastric emptying of liquids and small bowel transit of liquids were determined by means of radionuclide techniques. Gastric emptying of solids and colonic transit were measured by following transit of radiopaque spheres instilled into the stomach and proximal colon, respectively. RESULTS: With each dog as its own control, gastric emptying returned to preoperative values by 2 days in group 1, 3 days in group 2, and 5 days in group 3. Small bowel transit returned to preoperative values on day 3 for groups 1 and 2 and on day 5 for group 3. Gastric emptying of solids and colonic transit returned to preoperative values on day 3 for group 1 and on day 5 for groups 2 and 3. CONCLUSIONS: These observations show that laparoscopic resection techniques in this canine model resulted in a more rapid return to normal gastrointestinal transit compared with open colectomy.     

Clinical and serological courses of a newborn with post-transfusion hepatitis C

The clinical and serological course of a haemophilic baby who was transfused with 160 ml of blood containing the hepatitis C virus (HCV) (0.70 Meq. ml-1) on the sixth-post-natal day is described. He is the infant of an HCV-negative mother. One month after the transfusion, there was a marked increase in HCV RNA and a small amount of HCV antibody was detected. This case provides evidence that a newborn is capable of producing HCV antibodies.     

Relation between molecular structure and electrorheological effects in liquid crystalline polymers

The influence of the molecular structure of homogeneous fluids comprised of liquid crystalline polysiloxanes (LCSs) diluted in polydimethylsiloxane (DMS), on the increase in their viscosity upon application of an electric field (electrorheological effect, or the ER effect) was investigated, particularly in relation to the degree of polysiloxane polymerization, the composition of their mesogenic side chains, and the composition of alkyl spacers inserted between these two components. A large ER effect was observed in LCSs having spacer length of 3 carbons (C3) and diluted with DMS, even though little such effect was observed with no DMS dilution. The ER effect generally increased with increasing DMS dilution and with increasing mesogenic group content, main chain length, and spacer length. At the same time, however, the miscibility of the DMS solvent with the LC polymer decreased, resulting in instability and difficulty in measuring the shear stress at temperatures up to 40°C or higher. When spacers containing an ether bond were employed, on the other hand, little or no polymer-solvent phase separation occurred, and the ER effect was strong and stable down to temperatures of 30°C or lower. With all of the LCSs, the ER effect was observed at temperatures above their isotropic phase transition temperature as measured by DSC. © 1997 John Wiley & Sons, Inc. Chemical composition of seeds from Cucumeropsis mannii Naudin and their suitability as food

Dehulled seeds from Cucumeropsis Mannii Naudin mainly consist of lipids (40.3%) and proteins (34.5%). Carbohydrates, minerals, and water amount to 16.5, 3.7, and 5.9%, resp. From this composition a caloric value of 2 190 kJ/100 g is calculated. The major component of the oil linoleic acid (57.9%). Short-chain fatty acids are absent. All important macro and micro nutrient elements are present in sufficient amounts for human nutrition. The seeds are rich in vitamin E and in niacin (30.8 mg/100 g and 14.3 mg/100 g. resp.). Consumption of 100 g dehulled seeds covers the daily requirement of essential fatty acids, vitamin E and essential amino acids--methionine excepted. Besides starch (14.3 g/100 g) sucrose (1.14 g/100 g), raffinose (0.42 g/100 g) and stachyose (0.41 g/100 g) as well as traces of glucose and fructose are present. The proteins extracted with various solvents (H2O, 0.1 M KCl-, 0.1 mM K3PO4-, and 0.5% SDS-solution) are studied by amino acid analysis. SDS-electrophoresis and isoelectric focusing. Molecular weights of these proteins are between 5,000 and 80,000 daltons with the fraction between 20,000 and 35,000 predominating. The seeds exhibit weak inhibition of trypsin and do not inhibit alpha-chymotrypsin.
