OBJECTIVE: To report a case of fatal pulmonary embolism associated with the use of i.v. estrogen therapy for menometrorrhagia. DESIGN: Case report. SETTING: University hospital. PATIENT(S): A 52-year-old woman with fibroid uterus treated with GnRH analogues with add-back therapy who presented with excessive vaginal bleeding. INTERVENTION(S): Intravenous conjugated estrogens were administered for a total of six doses. MAIN OUTCOME MEASURE(S): Fatal thromboembolic event. RESULT(S): The day after i.v. conjugated estrogens were administered, the patient had only scant vaginal bleeding, but she experienced the sudden onset of respiratory distress, became comatose, and subsequently had ventricular fibrillation leading to asystole. All resuscitative efforts failed. Postmortem examination revealed bilateral pulmonary artery thromboembolism (saddle embolus). CONCLUSION(S): Intravenous conjugated estrogen therapy may be complicated by fatal thromboembolic events. This potential adverse effect must be considered in the use of such therapy for severe menometrorrhagia, especially when treating a patient at increased risk. 相似文献
We recently reported on the immunolocalization of type II secretory nonpancreatic phospholipase A2 (snpPLA2) in human atherosclerotic lesions. In the present study, we present data on the distribution and ultrastructural localization of snpPLA2 in adjacent nonatherosclerotic and atherosclerotic regions of human arteries. Electron microscopy (EM) of immunogold labeling techniques with a monoclonal antibody was used to analyze arterial tissue. The human specimens analyzed were obtained from autopsy and surgery cases. The results with EM showed a stronger snpPLA2 immunoreactivity in regions of arteries with atherosclerotic lesions than in regions without lesions from the same individual. snpPLA2 immunoreactivity was stronger in the arterial intima of atherosclerotic than of nonatherosclerotic tissue. EM-immunogold examination revealed that the majority of snpPLA2 was localized along the extracellular matrix, associated with collagen fibers and other extracellular matrix structures. Intracellular snpPLA2 was observed in electron-dense vesicles in intimal cells. snpPLA2 was also found in contact with large, extracellular lipid droplets. These results support the hypothesis that extracellular snpPLA2 is localized at sites where it may hydrolyze phospholipids from lipoproteins and lipid aggregates retained in the extracellular matrix of the arterial wall. This may be a mechanism for in situ release of proinflammatory lipids, free fatty acids, and lysophosphatidylcholine in regions of apolipoprotein B accumulation, which are abundant in atherosclerotic lesions. 相似文献

A study has been made of the effects of pigeon pea starch obtained from the plant Cajanus cajan (L) Millisp. (family Fabaceae) and plantain starch obtained from the unripe fruit of Musa paradisiaca L. (family Musaceae) on the compressional, mechanical, and disintegration properties of paracetamol tablets in comparison with official corn starch BP. Analysis of compressional properties was done by using density measurements, and the Heckel and Kawakita equations, whereas the mechanical properties of the tablets were evaluated by using tensile strength (T--a measure of bond strength) and brittle fracture index (BFI--a measure of lamination tendency). The ranking for the mean yield pressure, P(y), for the formulations containing the different starches was generally corn < pigeon pea < plantain starch while the ranking for P(k), an inverse measure of the amount of plasticity, was pigeon pea < plantain < corn starch, which indicated that formulations containing corn starch generally exhibited the fastest onset of plastic deformation, whereas those formulations containing pigeon pea starch exhibited the highest amount of plastic deformation during tableting. The tensile strength of the tablets increased with increase in concentration of the starches while the Brittle Fracture Index decreased. The ranking for T was pigeon pea > plantain > corn starch while the ranking for BFI was corn > plantain > pigeon pea starch. The bonding capacity of the formulations was in general agreement with the tensile strength results. The disintegration time (DT) of the formulation increased with concentration of plantain and corn starches but decreased with concentration of pigeon pea starch. The general ranking of DT values was plantain < pigeon pea < corn starch. Notably, formulations containing pigeon pea starch exhibited the highest bond strength and lowest brittleness, suggesting the usefulness of pigeon pea starch in producing strong tablets with minimal lamination tendency. Plantain starch, on the other hand, would be more useful where faster disintegration of tablet is desired. The results show that the starches could be useful in various formulations depending on the intended use of the tablets with the implication that the experimental starches can be developed for commercial purposes. 相似文献
This study investigates the microstructure of the processed cassava leaves. Standard parts machined from low visible diffusion zone formed in low carbon steel pack cyanided using carbon steel were pack cyanided by high temperature in-situ diffusion of nascent carbon and nitrogen into austenite. Formation of a visible diffusion zone was confirmed after observation with a Reichert-Jung Polyval optical microscope coupled with Zeiss AxioCam ICc 1 camera. High magnification Scanning Electron Microscope (SEM) imaging of the visible diffusion zone was done with a CamScan Series 2 scanning electron microscope. The zone was found variable in depth as measured from a representative position. The structure of all samples consists of ferrite and pearlite as determined by optical and SEM examination. Resulting products were thus both tough and hard suitable for service environments requiring high resistance to both wear and impact failure. 相似文献
The purpose of this study was to elucidate functional properties of starch granules obtained from tubers of Tacca leontopetaloides and compare them to a commercially available maize starch. Scanning electron microscopy (SEM), particle size analysis, X‐ray powder diffraction (XRPD), gravimetric moisture sorption, and differential scanning calorimetry (DSC) were used to characterize the samples. Tacca starch exhibited a monomodal distribution of irregularly shaped granules with a mean particle size of 2.64 µm. The spherulites of both samples indicated an A‐type pattern, with the degree of crystallinity estimated to be 35% for tacca starch and 38% for maize starch. The moisture sorption profile of both samples was analyzed according to the Guggenheim, Anderson and de Boer (GAB) equation. GAB analysis estimated the monolayer coverage for tacca and maize starch to be 0.0928 g/g and 0.0856 g/g, respectively. The gelatinization parameters of tacca starch were found to be 65.57 – 68.56 – 73.10°C while that of maize starch were 67.30 – 70.97 – 76.25°C. The results of DSC studies indicate that the associative forces that stabilize the granule structure in tacca starch are weaker than those in maize starch. The results obtained in this study establish the fundamental characteristics of tacca starch and suggest that further exploration of its potential for use in a variety of fields is warranted. 相似文献
The paper summarises the results of a recently completed study on manpower requirements in a selected number of housing projects in Nigeria and compares the findings with known previous works from both Nigeria and other advanced countries. It shows that manpower requirements for housing in Nigeria vary from 4.05 to 6.87 man-days per square metre in two previous studies compared with 6.44 to 16.78 man-days per square metre and 55–133 man-days per 1000 obtained from the present study. Similar studies carried out elsewhere were shown to vary from an average of 2.33 man-days per square metre for U.K., 3.28 and 1.53 man-days per square metre for Ireland and U.S.A. respectively. It was evident therefore that the productivity of Nigerian workers was much lower than in these other countries. The paper attributes the lower manpower requirements obtained elsewhere to better organisation, higher degree of supervision and adoption of mechanical power as well as a fair amount of prefabrication outside the building sites. The paper showed that the very variability in performance of the Nigerian contractors was mainly due to the form of construction, the size of contractor and size of contract executed by them. It concludes by suggesting various ways in which the manpower requirements for capital projects can be minimised. 相似文献
A bioflocculant named MBF-UFH produced by a Bacillus species isolated from sediment samples of Algoa Bay of the Eastern Cape Province of South Africa was characterized. The bacterial identification was through 16S rDNA sequencing; nucleotide sequences were deposited in GenBank as Bacillus sp. AEMREG7 with Accession Number {"type":"entrez-nucleotide","attrs":{"text":"KP659187","term_id":"768700521","term_text":"KP659187"}}KP659187. The production of the bioflocculant was observed to be closely associated with cell growth. The bioflocculant had the highest flocculating activity of 83.2% after 72 h of cultivation, and approximately 1.6 g of purified MBF-UFH was recovered from 1 L of fermentation broth. Its chemical analyses indicated that it is a glycoprotein composed of polysaccharide (76%) and protein (14%). Fourier transform infrared spectroscopy (FTIR) revealed that it consisted of hydroxyl, amide, carboxyl and methoxyl as the functional moieties. Scanning electron microscopy (SEM) revealed the amorphous structure of MBF-UFH and flocculated kaolin clay particles. The maximum flocculating activity of 92.6% against kaolin clay suspension was achieved at 0.3 mg/mL over pH ranges of 3–11 with the peak flocculating rate at pH 8 in the presence of MgCl2. The bioflocculant retained high flocculating activity of 90% after heating at 100 °C for 1 h. MBF-UFH appears to have immense potential as an alternative to conventional chemical flocculants. 相似文献
