INTRODUCTION: In a retrospective study a treatment concept for Perthes' disease dependent on the containment was applied. PATIENTS/METHODS: 49 hips of 41 children (9 female, 32 male) were treated between 01. 01. 1990 and 31. 12. 1995. In our concept of treatment a varus femoral osteotomy was performed in 28 cases with not contained hips or less than 4/5 coverage of the femoral head (X-ray/MRI). The other 21 well contained hips with 4/5 coverage or more were treated conservatively with physiotherapy and in case of joint effusion and pain additionally with the use of crutches (partial weight bearing) and anti-inflammatory medication. The average age in the non-operative group at the time of first investigation was 4 years and 9 months (3 y./1 m. to 7 y./1 m.) and 6 years and 3 months (4 y/2 m. to 10 y/0 m.) at our last examination (mean follow up 17.7 months, range of 6 to 72 months). At the time of indication for a varus femoral osteotomy the patients had an average age 6 years and 1 month (3 y./6 m. to 10 y./2 m.), the mean age at the last postoperative examination was 7 years and 11 months (4 y./8 m. to 12 y./5 m.) with an average follow up of 21.5 months (6 to 77 months). RESULTS: For the conservatively treated children we achieved good results (still well contained hips with 4/5 coverage, no decrease of function, no increase of pain) in 85.7% (18 of 21 cases). In 85.7% (24 of 28 cases) we found good results (well contained hips, increase of coverage, no decrease of function, no increase of pain) in the operation group. CONCLUSIONS: The presented concept of therapy in Perthes' disease was practicable for all patients and included the possibility of decision for operative or non-operative treatment. In both groups we achieved good results in 85.7% of the cases.  相似文献

A three-dimensional surface is a useful graphic representation of a two-dimensional function which has been sampled on a regular grid. Shading the surface to simulate the effects of direct lighting makes visible small changes in the surface orientation, and enhances realism when the data represents a physical surface such as terrain. Shading interpolation calculations and surface patch generation techniques require the specification of a surface normal vector (or related slope information) at each sample point. These normal vectors are usually generated by averaging local data such as the normal vectors of the surfaces of a triangular mesh connecting the points. This paper describes a technique which uses Fourier filtering to generate normal vectors for two-dimensional sampled data. Images and analysis of frequency spectra are included to show how this technique preserves detail which is lost using the averaging method. Performance figures show that this enhancement of detail in the final image can be achieved for only a small increase in computation time.  相似文献

A between-side comparison of GABAA receptor subunit expression levels in the globus pallidus and anterior-pole motor thalamic nuclei of rats with an ibotenate lesion of the striatum, and rats receiving a fetal striatal graft in the lesioned area was made by using immunocytochemistry with subunit-specific antibodies, at different times post-lesion or different times post-grafting. At 10 days post-lesion, there was already an increase in the labeling of the alpha 1- and beta 2/3-subunits in the globus pallidus, entopeduncular nucleus and ventrolateral nucleus ipsilateral to the lesion when compared with the contralateral side, while there were no significant changes at the level of the ventromedial nucleus. Labeling of the alpha 2-subunit showed a clear increase in the entopeduncular nucleus compared with the contralateral side at 10 days post-lesion. Similar changes were also observed for the different subunits studied at 30 and 120 days after lesioning. Rats with 20-day old transplants of fetal striatal neurons that were implanted in the ibotenate lesioned striatum at 10 days post-lesioning, continued to show changes in the expression of GABAA receptor subunits, albeit at a lower level than those of ibotenate lesioned rats at similar age post-lesion. However, when examining rats with 70-day old transplants, the ibotenate-lesion induced between-side changes were almost completely compensated. These findings suggest a correlation between the maturation of the grafts and their capability to function in reestablishing neuronal circuits as shown by the reduction of changes in GABAergic transmission induced by ibotenate lesions, as indicated by the reversal of changes in GABAA receptor subunit in several areas of the basal ganglia circuit.  相似文献

Pneumocystis carinii is an important pulmonary pathogen responsible for morbidity and mortality in patients with AIDS. The acute-phase response (APR), the primary mechanism used by the body to restore homeostasis following infection, is characterized by increased levels of circulating fibrinogen (FBG). Although the liver is the primary site of increased FBG synthesis during the APR, we unexpectedly discovered that FBG is synthesized and secreted by lung alveolar epithelial cells in vitro during an inflammatory stimulus. Therefore, we sought to determine whether lung epithelial cells produce FBG in vivo using animal models of P. carinii pneumonia (PCP). Inflammation was noted by an influx of macrophages to P. carinii-infected alveoli. Northern hybridization revealed that gamma-FBG mRNA increased two- to fivefold in P. carinii-infected lung tissue, while RNA in situ hybridization demonstrated increased levels of gamma-FBG mRNA in the lung epithelium. Immunoelectron microscopy detected lung epithelial cell-specific production of FBG, suggesting induction of a localized inflammatory response resembling the APR. A systemic APR was confirmed by a two- to fivefold upregulation of the levels of hepatic gamma-FBG mRNA in animals with PCP, resulting in a corresponding increase in levels of FBG in plasma. Furthermore, immunoelectron microscopy revealed the presence of FBG at the junction of cell membranes of trophic forms of P. carinii organisms aggregated along the alveolar epithelium. These results implicate FBG in the pathogenesis of PCP in a manner similar to that of the adhesive glycoproteins fibronectin and vitronectin, which are known to participate in intra-alveolar aggregation of organisms and adherence of P. carinii to the lung epithelium.  相似文献

PURPOSE: The purpose of this experimental study was to evaluate the initial effect of a frozen-stored disc allograft on the osteoarthritic sheep temporomandibular joint (TMJ). MATERIALS AND METHODS: Bilateral osteoarthritis was induced in the TMJ of four sheep. Three months later, unilateral discectomy and frozen-stored disc allograft replacement were performed. The donor disc had been obtained from a healthy animal at killing. The disc was stored in a deep freezer at -70 degrees C for 1 month and thawed just before grafting. The contralateral joints were undisturbed and served as a control at 3 months. At 6 months after the initial procedure, 3 months after the repair, the four sheep were killed. RESULTS: All four grafted discs were perforated and displaced. In the repaired joints, the condylar articular surface was partly denuded in some areas, and part of the surface showed fibrous repair. The control joints showed osteoarthritic changes. CONCLUSION: Frozen-stored disc allograft is not a satisfactory interpositional material in the osteoarthritic sheep TMJ.  相似文献

A successful cutting-edge semiconductor manufacturer applies Lean principles with the goal set on perfecting its operation flow. In 2007 SEMI adopted a new standard E94-1107 that breaks the dependency between carrier and lot for processing wafers and formally introduces material redirection, which creates several opportunities for waste reductions and improved WIP handling. As depicted from simulations, the new standard is impacting several KPIs including throughput, cycle time, yield, and Just-In-Time customer response. This paper discusses different lean optimizations leveraging the new standard. Some tools require loading of many wafers simultaneously to achieve a high throughput. Although with reducing lot sizes, takt time can be improved, the sequential nature of these tools become limited by the number of load ports and thus have a negative impact on the equipment effectiveness. Before the new standard implementation, no carrier could be removed from load ports during processing to allow other carriers to load or unload wafers leaving the ports blocked. With this change, we can now remove the carrier from the Load Port after unloading wafers into the tool to allow filling the tool with the optimum number of wafers. Over the years, numerous lean manufacturing studies have been performed in terms of determining the correct lot size (number of wafers in a lot) to meet the Just-In-Time targets. Simulation runs demonstrate that smaller lots have up to 33% and 50% improved cycle time for batch tools and single wafer tools, respectively. Based on fluctuations in demand, there may not only be a need to speed up, but also to slow down production quickly. All these have direct correlation with scheduling of right sized lots (dynamic lot sizing), carefully planned delivery of carriers for processing and improved pull production techniques.  相似文献
