BACKGROUND: The goal of this study was to compare patterns of recurrence and long-term outcome after sphincter-saving procedures (SSPs) and abdominoperineal resection (APR) in patients with tumors located in the lower third of the rectum. METHODS: We reviewed the charts of 1001 patients operated on for primary rectal adenocarcinoma between 1980 and 1991. All patients with tumors located between 5 and 7 cm from the anal verge and treated with curative intent were included. RESULTS: Of the 261 patients who met our criteria, 162 had undergone SSP and 99 had undergone APR. The local recurrence rates for SSP and APR were 8% and 11%, respectively (p = 0.41), and the distant metastases rates were 23% and 28%, respectively (p = 0.35). Recurrence and distant metastases rates for SSP and APR, respectively, did not differ by TNM classification: state I, 10% versus 9% (p = 0.9); stage II, 25% versus 43% (p = 0.13); and stage III, 56% versus 57% (p = 0.92). Five-year disease-free survival rates for SSP and APR patients were 70.5% and 62.3%, respectively (p = 0.2). CONCLUSIONS: Tumors in the lower third of the rectum can be treated with sphincter-saving procedures without compromising the chance of cure.  相似文献

A computer model was extended and adapted to simulate the hygrothermal behavior of building envelope-wood components. The model was used to predict moisture movement in wood planks forming the decks of nonvented flat roofs insulated with cellulose. The gradient of water potential was considered as the driving force for moisture movement in wood. The model required the determination of convective heat- and mass-transfer coefficients, the sorption curves, the effective water conductivity for different wood species, and the hygrothermal conditions within the assembly to characterize the mass-conservation equation. Once these parameters were integrated in the computer model, this approach was then validated by carrying a simulation of the drying process of wood planks using experimental data from a large-scale test.  相似文献

Summary In the present paper we point out that the correct way to solve the Blasius problem by numerical means is to reformulate it as free boundary value problem. In the new formulation the truncated boundary (instead of infinity) is the unknown free boundary and it has to be determined as part of the numerical solution. Taking into account the partial inavariance of the mathematical model at hand with respect to a stretching group we define a non-iterative transformation method. Further, we compare the improved numerical results, obtained by the method in point, with analytical and numerical ones. Moreover, the numerical results confirm that the question of accuracy depends on the value of the free boundary. Therefore, this indicates that boundary value problems with a boundary condition at infinity should be numerically reformulated as free boundary value problems.  相似文献

This letter presents an analytical framework of the cell stay time (CST) in wireless LAN environment. CST is an important parameter that can be used to estimate how long a mobile user will stay in a particular cell and how many cells he will probably visit during his call holding time. CST can be also used to make predictive advanced reservations. A probability density function (p.d.f.) of the CST is obtained and an analytical expression of its main parameters such as the average and standard deviation values is outlined as a function of the average mobile user speed and cell radius  相似文献

This paper presents a method for the field measurement of airtightness for two-?and three-unit buildings using only one fan pressurization system, commonly referred to as a blower door. The main advantage of this single-fan method is its lower equipment and labor cost as compared with multiple-fan methods. Results from consecutive tests in the individual units are used to prepare a set of simultaneous equations for airflow, allowing for the calculation of the air leakage characteristics of both the interior partitions and the exterior envelope on a unit-per-unit basis. Tests were performed on four buildings using the single-fan method and were compared with those obtained with a two-fan method. Test results indicate that the single-fan method may be of value as an inexpensive method of estimating the airtightness of two-?and three-unit residential buildings.  相似文献

Architecture firms face an increasingly uncertain marketplace. Firm principals and leaders need to increasingly cultivate and develop the talents of firm members if the organization itself is to adapt to changes in building technologies, the activity of other design professions, and changing client tastes and capacities. Divided into five sections, the following article presents a framework for examining this challenge. The first section examines the broader marketplace context that firms face, and the second presents a framework for linking skills to human resource development tools. Arguing that development tools will fail if the firm's culture cannot support them, the third section examines how the ethic of collegiality and an artistic sensibility shape the firm's cultural infrastructure and make it difficult to implement a formal human resource development program. The fourth section presents a model of the firm that can overcome these obstacles, and the fifth summarizes the overall argument.  相似文献

Defects of the peripheral nervous system are extremely frequent in trauma and surgeries and have high socioeconomic costs. If the direct suture of a lesion is not possible, i.e., nerve gap > 2 cm, it is necessary to use grafts. While the gold standard is the autograft, it has disadvantages related to its harvesting, with an inevitable functional deficit and further morbidity. An alternative to autografting is represented by the acellular nerve allograft (ANA), which avoids disadvantages of autograft harvesting and fresh allograft rejection. In this research, the authors intend to transfer to human nerves a novel technique, previously implemented in animal models, to decellularize nerves. The new method is based on soaking the nerve tissues in decellularizing solutions while associating ultrasounds and freeze–thaw cycles. It is performed without interrupting the sterility chain, so that the new graft may not require post-production γ-ray irradiation, which is suspected to affect the structural and functional quality of tissues. The new method is rapid, safe, and inexpensive if compared with available commercial ANAs. Histology and immunohistochemistry have been adopted to evaluate the new decellularized nerves. The study shows that the new method can be applied to human nerve samples, obtaining similar, and, sometimes better, results compared with the chosen control method, the Hudson technique.  相似文献

In this paper, the tensile, fatigue and creep properties of the Ferritic/Martensitic (F/M) steel T91 and of the Austenitic Stainless (AS) Steel 316L in lead-bismuth eutectic (LBE) or lead, obtained in the different organizations participating to the EUROTRANS-DEMETRA project are reviewed. The results show a remarkable consistency, referring to the variety of metallurgical and surface state conditions studied. Liquid Metal Embrittlement (LME) effects are shown, remarkable on heat-treated hardened T91 and also on corroded T91 after long-term exposure to low oxygen containing Liquid Metal (LM), but hardly visible on passive or oxidized smooth T91 specimens. For T91, the ductility trough was estimated, starting just above the melting point of the embrittler (TM,E = 123.5 °C for LBE, 327 °C for lead) with the ductility recovery found at 425 °C. LME effects are weaker on 316L AS steel. Liquid Metal Assisted Creep (LMAC) effects are reported for the T91/LBE system at 550 °C, and for the T91/lead system at 525 °C. Today, if the study of the LME effects on T91 and 316L in LBE or lead can be considered well documented, in contrast, complementary investigations are necessary in order to quantify the LMAC effects in these systems, and determine rigorously the threshold creep conditions.  相似文献

Although moisture significantly affects the performance and durability of building envelope systems, effective methods to quantify the relative drying capacity of these systems are yet to be developed. A new testing method and an evaluation approach for comparing the drying capacity of wood-frame wall systems in evacuating water due to rain penetration in the stud cavities are presented in this paper. A controllable and consistent moisture loading is created by placing a water tray on a load cell at the bottom of the stud cavity of the wall assembly which is then subjected to lab generated indoor/outdoor conditions. The data on water evaporation from water trays and the monitored moisture accumulations in the materials surrounding the stud cavities are used to establish load–response relations. Using these relations the relative performance of various building envelope systems in preventing biodeterioration caused by rainwater penetration into the stud cavities can be compared. The concept of in-cavity evaporation allowance (ICEA) has been proposed and it is based on the limit of 20% moisture content (MC) being reached at any location of the building envelope.  相似文献

In Noirmoutier, a French island off the Atlantic coast, secondary effluents flow into a series of four lagoons, 1.4-2.8 m deep, and are reused for agricultural irrigation. The excess water is disposed of to the sea. The aim of this study was to provide a model capable of predicting the microbiological quality of the water pumped for irrigation or discharged to the sea. Meteorological variables, flow rates, physical-chemical characteristics and faecal coliform (FC) contents were monitored for a year and a half. The hydraulic pattern of each lagoon was assumed to be that of completely mixed reactor because of the calculated dispersion numbers and the wind mixing effect. Coliform decay was assumed to follow first order kinetics in each lagoon. Die-off coefficients were calculated in each lagoon using a non-steady-state model. The main bacterial removal mechanism was shown to be solar irradiation. Empirical equations were established to calculate die-off coefficients as a function of received solar energy and temperature. FC die-off rates were higher in the first lagoon and then decreased successively in those following. FC numbers in the different lagoons were predicted with reasonable accuracy in spite of high variation in inlet water quality. The model will facilitate the prediction of water quality under various climatic conditions and different water reuse scenarios and will help to optimise reclamation and storage facilities.  相似文献
