The results of complex social and ecologic study in the city of Ufa

The authors present results of social and ecologic study carried out in Ufa. The results describe the course of ecologically important processes and phenomena in correlation with the public subjective understanding. Public health is influenced by hygienic parameters of food and water quality as well as a complex of social, economic and psychologic factors. Weak correlation between those parameters and ambient air pollution necessitates more accurate approach to ecologic mapping of cities and to manipulation with data on lower atmosphere pollution with chemical hazards.     

The intra-aortic antibacterial therapy of wounded patients with gunshot meningoencephalitis

The original decision of permanent introduction of antibacterial means to tissues of brain at gunshot meningoencephalitis is offered. For antibacterial therapy the intra-aortal catheter with diameter of 2,5 mm (through a.femoralis) was introduced. After washing the catheter by solution of crystalloid with heparin the various combinations of preparations in 5% solution of glucose were introduced: cephalosporin--8 g/day; hentamicin and brumacilin--240 mg/day accordingly, amicacin--1500 mg/day. Speed of introduction--20-50 mg/h, total volume--500 ml. The catheter was in aorta not more than 10 days, maximum--14 days. A described technique was applied in Burdenko Main Military Clinical Hospital on 34 wounded in head. At computer tomography of brain of all wounded intracranially the splinters and bullets were revealed, clinically--meningoencephalitis. Foreign bodies have been extracted after cupping of clinical and laboratory signs of meningoencephalitis. The authors consider, that the technique is effective not only at wounds of brain, but also at suppurative meningoencephalitis of other etiology.     

Changes in blood pressure 24 hour monitoring data and quality of life in patients with systolic hypertension treated with arifon

AIM: The study of quality of life and 24-h monitoring blood pressure data in elderly patients with isolated systolic hypertension (SH) on arifon monotherapy. MATERIALS AND METHODS: 22 patients over 65 (mean age 69.8 +/- 1.3 years) suffering from SH entered the open trial. 24-h monitoring of blood pressure was performed at least 2 weeks after discontinuation of the previous antihypertensive treatment and after 4 weeks of arifon monotherapy (2.5 mg once a day). Quality of life was evaluated according to the standard SIP questionnaire. RESULTS: Arifon 4-week treatment resulted in a significant fall of systolic pressure for 24 hours, day and night (by 16.2, 13.7 and 17.4%, respectively; p < 0.001). Variability of blood pressure did not change much. 24-h index for systolic and diastolic blood pressure increased 2-fold and by 46%, respectively. Moreover, the rate of the systolic pressure growth in the morning hours decreased by 36.7% (p < 0.05). Psychosomatic status by SIP questionnaire changed for the best. CONCLUSION: Arifon is an effective treatment of SH and improves quality of life.     

The effect of a two-week anti-orthostatic hypokinesia on the cardiac function and on the latter''s regulation in monkeys

Medical practice guidelines are being developed rapidly and advocated as a crucial component in transforming and improving the delivery of health care services. However, little is known about the ability of hospitals to institute guidelines outside of experimental settings. This article approaches guideline implementation from the viewpoint of process innovation assimilation and presents specific recommendations for organizational tactics leading to successful implementation of practice guidelines.     

Optical elements for focusing of terahertz laser radiation in a given two-dimensional domain

Binary silicon-based diffractive optical element (DOE)—Gaussian-to-Square focuser (diameter of aperture is 30 mm) for the terahertz spectral range has been designed and characterized using terahertz radiation of the Novosibirsk Free Electron Laser (NovoFEL) at the wavelength of 141 μm. The preliminary experiments have demonstrated feasibility of application of binary silicon DOE for focusing of terahertz radiation into pre-given focal domain.     

Fluorination of pressure-polymerized C60 phases

The reactivity of O-, T- and R-phases of the high pressure-high temperature (HPHT) polymerized C₆₀ towards gaseous fluorine in the temperature range of 50-250 °C was investigated. The reaction products were characterized by FTIR, powder X-ray diffraction, SEM, EDX, and VTP-EIMS to determine the bulk stoichiometries, bonding patterns, phase compositions, crystalline structures and thermal decomposition behavior of the fluorinated polymers. At 1 h isothermal treatment duration, fluorinated products with various bulk stoichiometries were obtained from different polymer phases with the R-phase showing the highest fluorine uptake. At 250 °C, all C₆₀ polymers showed partial decomposition to unfluorinated C₆₀ monomer under fluorine atmosphere. At 200 °C, the fluorination of R-phase yielded a pure fluoropolymer most likely having a {C₆₀F_x}_n (x = 36-44) composition. The same fluoropolymer was presumably obtained from O- and T-phases in lower yields. The linear chain structure was suggested for this new fluorocarbon polymer in agreement with the molecular mechanics modeling calculations.     

Percutaneous effects of iodine inhibit the development of experimental silicosis

Experiments on white rats proved that the silicotic pulmonary changes slowed with the tails periodically immersed into iodine solution. Those pulmonary changes were due to inhalation of quartzite dust and especially due to intratracheal administration of standardized quartz dust DQ12. Influenced by inhalation of dust containing quartz, the rats demonstrated characteristic alterations in cytologic features of bronchial and alveolar lavage water. The "iodine baths" also declined those alterations, that may prove higher resistance of pulmonary macrophages to cytotoxic effects of quartz. This hypothesis finds a support in experiments with culture of rat peritoneal macrophages. If incubated in a medium with low iodine concentrations and subsequently washed off, the macrophages demonstrate lower incidence of cytotoxic effects due to the next incubation with particles of DQ12. The macrophage's higher resistance against cytotoxic agents is believed to result directly in antisilicotic effect of iodine, so "iodine baths" could be recommended for pathogenetic therapy and medical prophylaxis of silicosis.     

Structure and properties of hybrid composites based on hydroxyethyl cellulose and laminar alumosilicate

Alumosilicate particles are immobilized into the hydroxyethyl cellulose matrix by mechanical dispersion; polymeric film materials are obtained. The effect of concentration of an inorganic filling agent on the structure of hybrid composites is revealed. The resulting composites exhibit an antimicrobial effect with respect to Escherichia coli and Staphylococcus aureus bacteria.
