Carcinoma of the endometrium is the most common gynecologic malignancy. The majority of women present with stage I disease, and the most common presenting symptom is postmenopausal bleeding. Early detection of endometrial cancer is important, because up to 90 percent of patients with stage I disease can be successfully treated. Certain risk factors, such as obesity, hypertension and diabetes mellitus, are associated with the development of this malignancy. Office endometrial sampling has a sensitivity of up to 97 percent for diagnosing carcinoma of the endometrium and can often eliminate the need for dilatation and curettage. Endometrial cancer is treated by total abdominal hysterectomy, bilateral salpingo-oophorectomy and, in many cases, postoperative radiation therapy.  相似文献

The environmental footprint of housing is greatly influenced by the size of a dwelling. Housing size is the result of households’ dwelling selections; accordingly, it is critical to consider residential preferences and choices to inform efforts towards housing sustainability. This study aimed to understand tenants’ preferences for and choices of housing size as one amongst several dwelling characteristics and identify obstacles and opportunities for reducing size in the light of promoting sustainable housing. We employed logistic regression models to analyse a survey with 878 Swiss tenants, and our results identify preference for large dwellings as a major obstacle for reducing dwelling size among affluent tenants. Conversely, tenants with lower income might be forced to move to a smaller dwelling due to financial constraints or attribute higher importance to the financial benefit of lower rents. However, financial disincentives along with substantial non-monetary costs of moving, such as the disruption of local bonds and the difficulty of finding a satisfactory dwelling, can outweigh the benefits of moving to a smaller dwelling. To overcome such obstacles, we suggest offering incentives and other facilitating measures for downsizing moves as well as ensuring an adequate supply of smaller dwellings capable of providing high living quality. We highlight the potential of studying housing functions to conceptualize dwellings fulfilling these requirements.

Sprouting induces activation and de novo synthesis of hydrolytic enzymes that make nutrients available for plant growth and development. Consumption of sprouted grains is suggested to be beneficial for human health. Positive consumer perceptions about sprouted cereals have resulted in new food and beverage product launches. However, because there is no generally accepted definition of “sprouting,” it is unclear when grains are to be called sprouted. Moreover, guidelines about how much sprouted grain material food products should contain to exert health benefits are currently lacking. Accordingly, there is no regulatory base to develop appropriate food labeling for “sprouted foods.” This review describes the nutritional and technological properties of sprouted grains in relation to processing conditions and provides guidelines to optimize sprouting practices in order to maximize nutritive value. Relatively long sprouting times (3 to 5 days) and/or high processing temperatures (25 to 35 °C) are needed to maximize the de novo synthesis and/or release of plant bioactive compounds. Nutrient compositional changes resulting from sprouting are often associated with health benefits. However, supportive data from clinical studies are very scarce, and at present it is impossible to draw any conclusion on health benefits of sprouted cereals. Finally, grains sprouted under the above‐mentioned conditions are generally unfit for use in traditional food processing and it is challenging to use sprouted grains as ingredients without compromising their nutrient content. The present review provides a basis for better defining what “sprouting” is, and to help further research and development efforts in this field as well as future food regulations development.  相似文献

Reviews attachment-related studies of early aggression to show that aggressive behavior toward peers is related to disorganized or controlling patterns of attachment behavior toward parents but not to avoidant or ambivalent patterns. Longitudinal attachment studies indicate that risk factors identified in cross-sectional studies of aggressive school-age children, such as family adversity, parental hostility, parental depression, and child cognitive deficits, are already evident in infancy and predictive of later aggression, before the onset of coercive child behavior. In infancy, these risk factors are associated with disorganized attachment behaviors toward the caregiver characterized by signs of fear or dysphoria, irresolvable conflict between opposing behavioral tendencies, and elevated cortisol levels after separation.
