Anti-D in a D-positive renal transplant patient

PURPOSE: We report a case of postoperative reparalysis in the recovery room, following nebulized epinephrine. The patient was pharmacologically reversed with edrophonium after paralysis with rocuronium. CLINICAL FINDINGS: A 12-yr-old girl developed postoperative reparalysis following the intraoperative administration of rocuronium. A total of 0.92 mg.kg-1 rocuronium was administered. After surgery, pharmacological reversal was achieved with 20 mg edrophonium with 0.15 mg atropine sulfate iv 35 min after the last administration of rocuronium. Muscular relaxation was monitored using an ulnar peripheral nerve stimulator (PNS). After reversal, a full train-of-four and sustained tetanus at 50 Hz were present. In the recovery room, following nebulized epinephrine, the patient became apneic. The patient was paralyzed and an ulnar PNS demonstrated only one faint twitch. The paralysis was reversed with 1.5 mg neostigmine with 0.3 mg glycopyrrolate. CONCLUSION: Postoperative reparalysis following rocuronium may be a cause of postoperative respiratory distress. The definitive diagnosis is made using PNS and observing the response to pharmacological reversal. Nebulized epinephrine may have a previously undescribed role in the development of postoperative reparalysis.     

Calorimetric determination of the enthalpy change for the binding of methyl orange to poly(vinylpyrrolidone) in aqueous solution

Thermodynamic parameters have been determined for the interaction of methyl orange and poly(vinylpyrrolidone) in aqueous solutions containing tris, tris and hydrochloric acid, or water only. Enthalpy changes were determined by use of a flow microcalorimeter. Free energy changes were calculated from the results of equilbrium dialysis studies. The results are interpreted in terms of the behaviour of poly(vinylpyrrolidone) in these systems. Analysis of data shows that the hydrophobic interactions in the systems are in the order: tris > wateronly > tris-HC1.     

Postural support strategies of disabled drivers and the effectiveness of postural support aids

The paper discusses a series of driving trials that were conducted to investigate postural stability of disabled drivers and to assess the effectiveness of a representative sample of support aids. Twenty-three disabled drivers with varying levels of physical disability and seven non disabled drivers participated in the study. The test car was equipped with transducers to measure vehicle velocity and acceleration (longitudinal and lateral), steering wheel movement and torque, and the bracing forces exerted by the driver on the steering wheel. Video cameras were installed to record postural support strategies and displacement of the driver and to record deviation of the car from the specified path. Subjective data regarding driver attitudes and acceptance were also collected through the administration of questionnaires. Findings from the study showed that support aids significantly improved driving performance and reduced physical exertion to maintain an upright driving position for disabled drivers. However, ergonomics design aspects regarding the ease of use and acceptance of the support aids by the end users were identified as obstacles to their sustained use in everyday driving.

Effect of Tungsten and Molybdenum Doping on the Semiconductor–Metallic Transition in Vanadium Dioxide Produced by Evaporative Decomposition of Solutions and Hydrogen Reduction

V_{1– x} W _x O₂ and V_{1– x} Mo _x O₂, 0 < x < 0.03, powders have been produced by the evaporative decomposition of solutions of vanadyl sulfate hydrate with tungsten dioxide dichloride or molybdenum dioxide dichloride in a hydrogen/nitrogen atmosphere. The powders consist primarily of hollow, porous, spherical shells. Differential scanning calorimetric analysis of the powders indicates that the 67°C mono-clinic-tetragonal phase transition in VO₂ shifts to lower temperatures by 23°C/(at.% tungsten) and 6.3°C/(at.% molybdenum).     

Making mistakes when predicting shifts in species range in response to global warming

Many attempts to predict the biotic responses to climate change rely on the 'climate envelope' approach, in which the current distribution of a species is mapped in climate-space and then, if the position of that climate-space changes, the distribution of the species is predicted to shift accordingly. The flaw in this approach is that distributions of species also reflect the influence of interactions with other species, so predictions based on climate envelopes may be very misleading if the interactions between species are altered by climate change. An additional problem is that current distributions may be the result of sources and sinks, in which species appear to thrive in places where they really persist only because individuals disperse into them from elsewhere. Here we use microcosm experiments on simple but realistic assemblages to show how misleading the climate envelope approach can be. We show that dispersal and interactions, which are important elements of population dynamics, must be included in predictions of biotic responses to climate change.     

The feeling of knowing experience in educable retarded children.

Required 50 educable retarded children to predict their recognition accuracy when recall failed. Ss were divided into 3 ability levels: (a) old Ss (mean CA 13.25 yrs, mean MA 10.5 yrs), (b) medium Ss (mean CA 11.25 yrs, mean MA 8.75 yrs), and (c) young Ss (mean CA 9 yrs 5 mo, mean MA 6 yrs 9 mo). Results indicate that old and medium Ss could reliably predict their recognition accuracy, suggesting sensitivity to their own feeling of knowing experience, but that the young Ss showed no evidence of this sensitivity. Even though the young Ss had difficulty predicting their recognition accuracy in advance they were able to estimate the success or failure of their responses after they had occurred. Results are discussed in terms of the complexity of the metamemory judgment required, and the advisability of evaluating metamemorial knowledge across several situations is emphasized. Predicting change in depression following renal transplantation: Effect of patient coping preferences.

Improvement in patient quality of life is a central goal of renal transplantation. This study examined the hypothesis that change in depression following transplantation would vary as a function of patient coping preferences. Sixty patients were assessed with the Krantz Health Opinion Survey and the Beck Depression Inventory while on the waiting list for a cadaveric renal transplant. Patients were reassessed approximately 12 months later. Among the 33 patients receiving a transplant during the follow-up period, those with a high preference for health-related information exhibited a substantial reduction in depression. In contrast, patients low in preference for information showed a slight increase in depression. Among the 27 patients who were not transplanted during the follow-up period, preference for information had no effect on depression. Patient differences in preference for behavioral involvement did not exert a significant effect on depression. (PsycINFO Database Record (c) 2010 APA, all rights reserved)     

Psychometric properties of the Geriatric Depression Scale among the institutionalized aged.

Validity and reliability of the Geriatric Depression Scale (GDS) were examined among 806 nursing home and congregate apartment residents (mean age 84 years). The scale was highly internally consistent and, although a clean factor structure emerged, basically unidimensional. Test-retest reliability at 1 mo was good, but correlations with staff-rated depression were only moderate. GDS-based classifications of respondents as suffering possible major, minor, or no depression were fairly consistent with those based on a symptom checklist and with clinical diagnoses. However, both the GDS and the checklist yielded high false negative rates vis à vis clinically diagnosed minor depression. There were no differences in reliability or validity for cognitively impaired and intact groups. GDS responses did not vary with age or length of institutionalization, but there was a positive correlation with functional disability and ill health.
