Based on a survey of 121 faculty members in programs accredited by the American Psychological Association, 65% reported practice activity. The authors agree that the problem involves a much more complex integrative task that goes beyond simply recognizing knowledge gained in practice settings as valid. TODAM2: A model for the storage and retrieval of item, associative, and serial-order information.

Presents an extended version of the convolution–correlation memory model TODAM (theory of distributed associative memory) that not only eliminates some of the inadequacies of previous versions but also provides a unified treatment of item, associative, and serial-order information. The chunking model extended the basic convolution–correlation formalism by using multiple convolutions, n-grams (multiple autoassociations of sums of item vectors), and chunks (sums of n-grams) to account for chunking and serial organization. TODAM2 extends the chunking model by including rn-grams (reduced n-grams), labels, and "lebals" (the involution or mirror image of a label) to provide a general model for episodic memory. For paired associates, it is assumed that Ss store only labeled n-grams and lebaled rn-grams. Norman Joseph Slamecka (1928-2003).

Presents an obituary for Norman J. Slamecka (1928-2003). Slamecka and his wife Jan were killed suddenly in a pedestrian accident on August 2, 2003, in Lewes, Delaware. He was a Marine veteran of the Korean War, and after a few years of practice as a clinical psychologist for the Veterans Administration, he accepted a faculty position at the University of Vermont in Burlington. He then taught at the State University of New York at Buffalo for a short while before coming to the University of Toronto in 1970, where he remained until his retirement in 1994. Development of an outcomes management program at an academic medical center

Donor CD4+ and CD8+ T cells mediate graft-vs.-leukemia (GVL) responses in the allogeneic bone marrow transplantation (alloBMT) setting. To evaluate the role of functional T cell subsets in the mediation of GVL, alloreactive donor CD4+ (Th1/Th2) and CD8+ (Tc1/Tc2) T cells of defined cytokine phenotype were generated by in vitro culture. A leukemia/transplantation model (B6 into B6C3F1; 1050 cGy host irradiation) was established using the bcr/abl-transfected myeloid leukemia line, 32Dp210 (P210; H-2k). Leukemia control mice (1X10(4) P210 cells per recipient) died at day 12.0 post-BMT. Recipients of the CD4+, Th1-type or CD8+, Tc1-type populations were conferred a survival advantage (death at 20.7 and 23.5 days post-BMT, respectively). In contrast, the CD4+, Th2-type population did not mediate GVL (death at 12.3 days). Furthermore, cell mixing experiments demonstrated that the Th2 subset abrogated both Th1- and Tc1-mediated GVL. The CD8+, Tc2 population, which secreted type II cytokines and lysed the P210 leukemia target in vitro, mediated GVL in some experiments; interestingly, the magnitude of Tc2-mediated GVL was inversely related to the level of interleukin-10 (IL-10) secreted in vitro by the Tc2 population. These studies therefore indicate that alloreactive T cells of type I phenotype maximally generate GVL, and that type I/type II interactions are an important consideration for allogeneic transplantation in the setting of leukemic hosts.

Promoting pollution prevention through community-industry dialogues: the good neighbor model in Minnesota

This article examines five attempts by communities to promote pollution prevention through direct negotiations with local manufacturing plants. These projects were Good Neighbor Dialogues spearheaded by Citizens for a Better Environment-Minnesota, an environmental advocacy organization. Three community-company partnerships (a container plant, a foundry, and a cabinet manufacturer) were successful and two (a munitions plant and a petroleum refinery) were not. Successful dialogues all shared certain characteristics: the company was open to negotiating with the community; there was an effective "champion" within the company; a skilled, independent facilitator served as moderator; community participants received independent technical assistance; and both the company and community understood the value of cooperative environmental decision making. Results suggest that Good Neighbor Dialogues can, under the right settings and circumstances, be an effective mechanism for building social capital by fostering greater understanding and trust between companies and communities. They offer the prospect of community-company partnerships that promote pollution prevention and other environmental improvements, while at the same time reinforcing and amplifying traditional pollution control strategies.     

A rapid interview protocol supporting patient-centered quality improvement: hearing the parent's voice in a pediatric cancer unit

BACKGROUND: The Institute of Medicine's 2001 report on quality delimits six dimensions of optimal care: safety, effectiveness, efficiency, timeliness, patient centeredness, and equity. In fall 2001 parents of pediatric cancer patients were interviewed to determine how well they thought these dimensions were addressed with respect to medication administration. Immediate goals were to identify system weaknesses and devise strategies to prevent future errors. A higher-order goal was to develop and demonstrate a model protocol for rapid-cycle interview assessments. METHODS: Hematology/oncology directors worked with a research expert to develop a semistructured interview protocol. After training, which included directed reading, oral instruction, and role-playing, a convenience sample of 20 English- and Spanish-speaking parents of inpatients was recruited. Parents were asked to characterize current medication administration practices and to describe problems that they had experienced or witnessed. Rapid content analysis techniques were used to identify issues of importance to the parents. FINDINGS: Parents' medication concerns centered on their children's comfort. Parents called for communication improvements, standardization of all nursing procedures and techniques, and a guide or an outline providing a clear understanding of what to expect when and from whom. Viewing these concerns in relation to the Institute of Medicine's quality domains allowed the department to frame an improvement action plan aligned with organizational and national priorities. IMPLICATIONS: With good supervision and limited focused training, inexperienced staff can successfully administer semistructured qualitative interviews and help analyze findings for rapid cycle improvement purposes. The protocol can be adapted for use in organizations interested in rapid qualitative assessments of patient and parent preferences.     

Atrioventricular nodal reentrant tachycardia in patients with ventriculo-atrial conduction block

OBJECTIVE: To demonstrate the reversibility of retrograde ventriculo-atrial block by isoproterenol in patients with atrioventricular nodal reentrant tachycardia (AVNRT). DESIGN: Three case reports and their electrophysiological features. PATIENTS: Three patients with documented or suspected paroxysmal supraventricular tachycardia. INTERVENTIONS: At routine electrophysiology study, no supraventricular tachycardia was inducible in the baseline state. Infusion of isoproterenol (1 to 5 micrograms/min) was given and stimulation procedures were repeated. RESULTS: At baseline, all three patients had discontinuous antegrade atrioventricular (AV) nodal conduction, but very poor (two patients) or absent (one patient) ventriculo atrial conduction prevented induction of AVNRT. During infusion of isoproterenol, retrograde conduction was enhanced so that 1:1 retrograde occurred to cycle lengths of 300, 340 and 260 ms. AVNRT was then inducible in all patients, reproducing their clinical symptoms. CONCLUSION: Absent or poor ventriculo-atrial conduction in patients with suspected AV node reentry does not preclude the development of tachycardia with sympathomimetic enhancement. Isoproterenol should be given to attempt reversal of retrograde block in these patients.
