OBJECTIVE: Thoracic injury remains a major source of morbidity and mortality in urban trauma centers. With the advent and increasing expertise in video assisted thoracic surgery, this modality has become an attractive alternative in the management of patients with thoracic injury. This report will review our experience with video assisted thoracic surgery at a level I trauma center and attempt to further delineate the indications for and timing of thoracoscopy in thoracic trauma. METHODS: We identified 16 patients who had undergone video assisted thoracic surgery following chest trauma between July 1991 and June 1994. There were 15 penetrating and one blunt trauma. All 16 patients were initially treated with tube thoracostomy. From 0-20 days post-injury, video assisted thoracic surgery was attempted with either diagnostic or therapeutic intentions. RESULTS: Twelve of the 16 patients (75%) had successful thoracoscopy. Three patients had diaphragmatic injury excluded and nine patients had successful evacuation of clotted hemothoraces. Evacuation of clotted hemothorax up to 7 days post-injury was safe and easily accomplished. Four patients (25%) had unsuccessful thoracoscopy and were converted to standard thoracotomy; failure was attributed to either suboptimal single lung ventilation or severe pleural inflammatory reaction. The only death in the entire group occurred 10 days after a thoracotomy for retained hemothorax. The median post-operative hospital stay following successful video assisted thoracic surgery was 3.5 days. CONCLUSIONS: Video assisted thoracic surgery can be utilized as an effective and safe method for the initial diagnostic evaluation and surgical management of stable patients with penetrating thoracic trauma.  相似文献

A liquid chromatographic (LC) method was developed to determine 5 benzoylureas--diflubenzuron, hexaflumuron, teflubenzuron, flufenozuron, and lufenuron--in peppers, tomatoes, eggplants, cucumbers, and oranges. Preparation of samples involve extraction with acetone and partitioning into dichloromethane-petroleum ether. A portion of this extract is cleaned up with a solid-phase extraction aminopropyl disposable column. With LC analysis using an RP-8-DB microbore column, acetonitrile-water (70 + 30, v/v) as mobile phase, and photodiode array detection at 254 nm, recovery and repeatability data were collected for the 5 benzoylureas on 4 vegetables and citrus in the range 0.04-2.0 mg/kg. Validated limits of detection and quantitation were 0.01 and 0.04 mg/kg, respectively. The method is reliable for routine analysis of vegetables and fruits.  相似文献

The functional status of striatal GABAA receptors appears to be inversely related to the magnitude of cocaine-induced behaviors. Exposure of striatum to antisense oligodeoxynucleotides (ASODNs) targeted to the mRNAs for the alpha 2 and the beta 3 subunits of the GABAA receptor should decrease expression of receptor proteins and therefore might be expected to increase cocaine sensitivity. ASODNs, scrambled ODNs or saline were injected into right lateral ventricle of rats and behavioral responses to cocaine were tested 18-20 h after treatment. Animals injected separately with alpha 2 or beta 3 ASODNs exhibited increased behavioral sensitivity to cocaine compared to rats injected with saline or scrambled ODNs including performing more 360 degrees turns to the left than to the right. There was significantly less GABA-stimulated Cl uptake in right striatum compared to left striatum of ASODN-treated rats with no significant difference between sides in control animals. Specific binding to benzodiazepine and convulsant sites on the GABAA receptor was not selectively altered by ASODN treatment. Combined alpha 2 beta 3 ASODN treatment did not affect either cocaine sensitivity or GABAA receptor function. There was no difference between the density of Nissl stained cells in the left and right edges of striatum in control or ASODN-treated rats indicating the absence of significant neurotoxic effects of the ASODN treatment. Injection of fluorescein-conjugated ASODNs indicated that ASODN is present in striatum at times during which behavioral and neurochemical indices of GABA receptor function are decreased. Thus, the functional status of GABAA receptors in striatum may be involved in determining cocaine sensitivity.  相似文献

Radio frequency identification (RFID) is a powerful technology that enables wireless information storage and control in an economical way. These properties have generated a wide range of applications in different areas. Due to economic and technological constrains, RFID devices are seriously limited, having small or even tiny computational capabilities. This issue is particularly challenging from the security point of view. Security protocols in RFID environments have to deal with strong computational limitations, and classical protocols cannot be used in this context. There have been several attempts to overcome these limitations in the form of new lightweight security protocols designed to be used in very constrained (sometimes called ultra‐lightweight) RFID environments. One of these proposals is the David–Prasad ultra‐lightweight authentication protocol. This protocol was successfully attacked using a cryptanalysis technique named Tango attack. The capacity of the attack depends on a set of boolean approximations. In this paper, we present an enhanced version of the Tango attack, named Genetic Tango attack, that uses Genetic Programming to design those approximations, easing the generation of automatic cryptanalysis and improving its power compared to a manually designed attack. Experimental results are given to illustrate the effectiveness of this new attack.  相似文献

During recent years, considerable effort has been devoted to the enhancement of Distributed Hash Table (DHT) systems with broadcasting capabilities. Such systems typically provide individual node routing but a broadcast primitive is required for functionalities such as information dissemination or data aggregation. Broadcasting can also be used as the basis for partial keyword searches. Little work has however specifically addressed Kademlia, a well known DHT, used in real applications. Our work exposes the particularities of this system, notably its XOR-based distance metrics, and analytically studies what broadcasting techniques can be applied to it. A model that estimates node coverage as a function of the probability that individual messages reach their destination has been also developed. For validation, several broadcasting algorithms have been implemented and comprehensively evaluated, considering node coverage, messages to nodes ratio, latency and imbalance factor. Moreover, several techniques are proposed to enhance the bare protocols when adverse circumstances such as churn and failure rate conditions are present. These include redundancy, resubmissions or flooding, and also combinations of those. All have been implemented and fully tested. An analysis of the strengths and weaknesses of algorithms and additional techniques, and a discussion on the choices and compromises to make, depending on system characteristics or application priorities, is finally presented.  相似文献

The aim of this work was to evaluate the effect of intramammary infection (IMI) on the endogenous proteolysis of milk. Four control checks were carried out in the half-udder milk of 10 ewes that acquired unilateral subclinical mastitis. Two of these checks were conducted before the infection was established and 2 after. Ten healthy ewes were tested as a control group. The presence of a subclinical IMI involved an increase of the products of casein hydrolysis, the proteose-peptone (p-p) fraction and minor (m) caseins, and a decrease of β-casein. As a result, a significant increase in the proteolysis index (PI), calculated as the ratio of m-casein to the sum of caseins (α + β + κ), took place. α-Casein and κ-casein were not significantly affected by IMI. Correlations confirmed the scenario: log10 of somatic cell count (SCC) was positively correlated with p-p content and negatively with β-casein, whereas log10 SCC was not correlated with α-casein or κ-casein. On the other hand, p-p content was positively correlated with m-casein and PI and negatively with β-casein, but no correlation was detected between p-p content and α- or κ-casein. Furthermore, between casein fractions, m-casein was only significantly correlated with β-casein. These results suggest that use of indices of proteolysis of caseins such as p-p, m-casein, and PI, could be applied together with SCC to evaluate the cheese-making quality of milk.  相似文献

In this investigation, the characteristics and the rheological properties of two different nanocomposite systems were investigated. These systems consisted of a dispersion of carbon nanotubes (CNTs) and carbon nanofibers (CNFs) in a polypropylene (PP) matrix. The mixing process was carried out by melt compounding with a twin‐screw corotating extruder with different reinforcement amounts (0.2–20 wt %) from concentrated masterbatches (20 wt %) of PP/CNT and PP/CNF. The results show a remarkable increase in the viscosity for both blends as the reinforcement amount was increased. It was important to evaluate the rheological behavior to understand the effect of the nanocarbon particles on the internal structures and their processing properties of the obtained composites. CNFs were a more viable reinforcement from a processability point of view because the obtained viscosities of the PP/CNF blends were more manageable. © 2012 Wiley Periodicals, Inc. In eleven patients with isolated mitral stenosis and regular sinus rhythm a right cardiac catheterization was performed and the wedged pulmonary capillary pressure recorded at rest and during electrical pacing of the right atrium at successive frequencies of 100, 120, 140, and, occasionally, 160 and 180 beats/min, while cardiac output was estimated by the Fick's principle. In all cases a significant elevation of pulmonary capillary pressure with a simultaneous reduction in cardiac output was obtained. The rise of wedged pulmonary pressure was proportional to the increment in cardiac frequency and related also to the calculated area of the mitral valve. The influence of active atrial contraction upon pulmonary pressure and cardiac output is discussed and comparisons with other studies are made. Emphasis is made on the value of atrial pacing as a diagnostic method in mitral stenosis, especially in cases in whom classical effort manoeuvres can not be applied or are insufficient to rise cardiac frequency.  相似文献

The presence of drug residues in ewe's milk samples can be determined by microbial assays. The main limitation of these tests is the large number of false-positive results associated with them. False-positive results can be explained by the interaction of certain substances naturally existing in ewe's milk with the growth of the microorganism used in the test. In this study, milk chemical composition (fat, protein, lactose, total solids), somatic cell counts (SCCs), free fatty acid concentrations, and lactoperoxidase system components were determined in order to investigate their influence on the rate of false-positive results for the BRT and Delvotest microbiological inhibitor tests. Milk samples were obtained after morning milking of Manchega ewes at 15, 30, 45, 60, 75, 90, 105, 120, and 135 days after parturition. The animals did not receive any kind of treatment or medicated feed throughout the experiment. The false-positive rates for BRT and Delvotest were 3.75 and 2.4%, respectively. When the logistic regression model was applied, the percentages of total solids for positive samples were significantly different from those for negative samples (16.90 versus 18.42% for BRT, 16.05 versus 18.45% for Delvotest), while the SCC logarithmic transformation was significantly higher for the positive samples than for the negative samples (5.38 versus 5.11 log units for BRT, 5.32 versus 5.11 log units for Delvotest). Moreover, Delvotest-positive samples exhibited thiocyanate concentrations higher than those of Delvotest-negative samples (8.18 mg/liter versus 6.85 mg/liter). Further analyses are needed to confirm the possible presence of antimicrobial residues in this particular type of milk sample.  相似文献

In this work on machine milking of ewes, pulsation rates of 120 and 180 cycles per min were compared, both with a pulsation ratio of 50:50 and a vacuum level of 36 kPa, comparing intramammary infection (IMI), somatic cell count (SCC) and teat end thickness changes. To this end, two groups of 20 Manchega ewes were used in a crossover experimental design with two experimental periods of 24 d for each. Bacterial exposure of all teats was increased by dipping them in a suspension of Staphylococcus simulans at four consecutive milkings of each period. Pulsation rate of 180 cycles per min, compared with 120 cycles per min, had no negative effect upon new IMI (11 and 16% of ewes infected, respectively) and SCC. No teat end lesions were observed in those animals milked with the two pulsations assayed. Also, teat thickness changes (-0.38 and -0.36 mm at 120 and 180 cycles per min, respectively) were not affected significantly. Finally, in absence of IMI, the two pulsation rates assayed did not affect the SCC.  相似文献
