The authors describe a case of one patient who underwent duodenocephalopancreasectomy for CBD cancer 6 years ago and developed gastrointestinal bleeding caused by a fistula between the hepatic artery and the jejunal stump. They discuss the differential diagnosis problem in the gastrointestinal bleeding syndrome caused by this rare pathology.  相似文献

BACKGROUND: Questionnaire-based surveys from several countries have consistently detected adverse health associated with home dampness and mould growth. METHODS: To test the validity of questions commonly used to indicate the presence of indoor mould, questionnaires were administered in 403 homes where dust samples were taken for viable fungi and air samples for ergosterol. RESULTS: Geometric mean concentrations of the total viable fungi were 255 (SE 116) x 10(3) CFU/g when mouldy odours were reported and 155 (SE 55) when odours were not reported (P = 0.01). Similarly, reported water damage was associated with a 50% increase (P = 0.06). Geometric mean concentrations of the predominantly indoor-source fungi, Aspergillus plus Penicillium, were twice as high when mould or mildew was reported than when not mentioned (P = 0.01). The presence of reported mould or water damage was unrelated to the presence of detectable levels of ergosterol. There was evidence for reporting bias: in the presence of low concentrations of viable fungi in dust, respondents reporting allergies were more likely to report visible mould growth (odds ratio [OR] = 1.8, 95% confidence interval [CI]: 0.9-3.5, P = 0.10. In the presence of elevated concentrations of dust fungi, respondents who smoked were less likely to report visible mould growth, (OR = 0.4, 95% CI: 0.2-0.7, P = 0.005). CONCLUSIONS: Reported mould, water damage, and mouldy odours were associated with elevated levels of indoor fungi. However, inaccuracy was high and there was evidence of a systematic reporting bias. Future research should concentrate on developing accurate objective measures of exposure to fungi, and then use this information to develop valid questionnaires. Currently, objective measures not questionnaires, are recommended to clarify the health effects of indoor fungi.  相似文献

Flora-bearing mice with targeted disruption of T cell receptor (TCR)-alpha or -beta genes spontaneously develop intestinal inflammation with features similar to ulcerative colitis in humans. TCR-alpha-deficient mice maintained germfree or colonized with a limited number of intestinal bacteria failed to develop inflammatory bowel disease (IBD)-like lesions. Evidently, inflammation in these mice does not develop spontaneously or result from a generalized antigenic stimulation, but rather requires induction by a heretofore unidentified specific stimulus. We describe the development of IBD-like lesions in germfree TCR-alpha-deficient mice monoassociated with the protozoan Cryptosporidium parvum. Lesions were seen in distal ileum, cecum, and colon and were most severe in the cecum. A prominent leukocytic infiltrate within the lamina propria was a common characteristic of the lesions observed in the C. parvum-infected germfree TCR-alpha-deficient mice. The leukocytic infiltrate was composed of aggregates of B220+ cells, the majority of which expressed surface IgD (ie, conventional B lymphocytes). It has been proposed that antigenic stimulation by a microorganism(s) is needed to initiate intestinal inflammation in TCR-alpha-deficient mice. Our results indicate that a single microbial species, C. parvum, is capable of triggering the development of IBD-like lesions in germfree TCR-alpha-deficient mice.  相似文献

Characterization of the lipid fraction of raw cow and buffalo milk samples, collected in different breeding areas in Apulia, a region of southern Italy, were performed by means of 1H NMR. The aim of this work was to establish whether FA composition data obtained by 1H NMR can be used in the differentiation of buffalo and cow milk samples according to species. A complete assignment of the signals present in the spectrum was attempted by COSY, heteronuclear coherence spectra. Quantification of FA was carried out by inserting the integrals of particular peaks in suitable calculations. Multivariate statistical analysis, conducted on the results of the quantification, permitted buffalo and cow milks to be distinguished.  相似文献

The main goal of this paper is to show how relatively minor modifications of well-known algorithms (in particular, back propagation) can dramatically increase the performance of an artificial neural network (ANN) for time series prediction. We denote our proposed sets of modifications as the 'self-momentum', 'Freud' and 'Jung' rules. In our opinion, they provide an example of an alternative approach to the design of learning strategies for ANNs, one that focuses on basic mathematical conceptualization rather than on formalism and demonstration. The complexity of actual prediction problems makes it necessary to experiment with modelling possibilities whose inherent mathematical properties are often not well understood yet. The problem of time series prediction in stock markets is a case in point. It is well known that asset price dynamics in financial markets are difficult to trace, let alone to predict with an operationally interesting degree of accuracy. We therefore take financial prediction as a meaningful test bed for the validation of our techniques. We discuss in some detail both the theoretical underpinnings of the technique and our case study about financial prediction, finding encouraging evidence that supports the theoretical and operational viability of our new ANN specifications. Ours is clearly only a preliminary step. Further developments of ANN architectures with more and more sophisticated 'learning to learn' characteristics are now under study and test.  相似文献

We present analytical and experimental results on fluxgate magnetometers that make use of a readout technique based on residence times. This approach allows for enhancing sensitivity to weak target signals in particular when the reduction of the sensor dimensions are considered. Our approach, exploiting the inherent nonlinear character of the bistable core dynamics, is based on the time domain characterization of the transitions between the two saturation states of the hysteresis loop that is inherent in the ferromagnetic core dynamics. This readout technique can be implemented with bias signals having lower amplitude and frequency than those used in conventional fluxgate processing schemes, thus reducing the device power requirements. The efficacy of this strategy is shown through an analytical approach and via experimental results which suggests guidelines for optimal device design and realization. The experiments have been carried out on a miniaturized laboratory fluxgate prototype; this device shows numerous desirable characteristics, including very good sensitivity and resolution, as well as ease of operation and a very low cost.  相似文献

Aim of the paper is to propose a new coupled interface-body damage model for the study of the detachment process in concrete or masonry structures strengthened with fiber reinforced polymers (FRP). In particular, a model of the FRP-concrete or -masonry interface, accounting for the coupling between the degradation of the cohesive material and the FRP detachment, is presented. To this end, a nonlocal damage model is considered for the quasi-brittle material. Regarding the interface, a model which accounts for damage, unilateral contact and friction is developed. The novelty of the proposed model consists in taking into account the coupling between the body and the interface damage, ensuring that the interface damage is not lower than the body one. Some numerical examples and a comparison with experimental data are presented in order to verify the efficiency of the proposed model in reproducing the FRP decohesion from the support cohesive material.  相似文献

Laser writing attached many attentions for fabrication micro-channels in microfluidics devices and lab-on-chip devices for biomedical applications. In this study, micro-channels were fabricated on different materials as masters using nanosecond diode-pumped solid state (DPSS) laser writing for imprinting on glass and polymer microfluidics devices. Good quality microstructures were fabricated on silicon, nickel alloy, cooper/brass and alumina, respectively by laser writing which proved that the nanosecond DPSS laser is suitable for rapid prototyping and rapid manufacturing of surface microstructures on different substrates as mask-less exposure system of imprinting.  相似文献

In this paper, a suitable model of distributed supply-chains (DSCs) is presented with the aim of providing a tool for DSC decentralised optimisation. To cope with this challenge, in the first part of the paper, a general model for distributed supply-chain including suppliers, processing units, assemblers, and transportation systems is presented with the aim of keeping the framework as general as possible. A stabilized finite point method (FPM) for the meshless analysis of incompressible fluid flow problems is presented. The stabilization approach is based in the finite increment calculus (FIC) procedure developed by O?ate [14]. An enhanced fractional step procedure allowing the semi-implicit numerical solution of incompressible fluids using the FPM is described.
