Implementing information and communications technology (ICT) at scale requires evaluation processes to capture the impact on users as well as the infrastructure into which it is being introduced. For older adults living with cognitive impairment, this requires evaluation that can accommodate different levels of cognitive impairment, alongside input from family and formal caregivers, plus stakeholder organisations. The European Horizon 2020 project INdependent LIving support Functions for the Elderly (IN LIFE) set out to integrate 17 technologies into a single digital platform for older people living with cognitive impairment plus their families, care providers and stakeholders. The IN LIFE evaluation took place across six national pilot sites to examine a number of variables including impact on the users, user acceptance of the individual services and the overall platform, plus the economic case for the IN LIFE platform. The results confirmed the interest and need among older adults, family caregivers, formal caregivers and stakeholders, for information and communications technology (ICT). Relative to the baseline, quality of life improved and cognition stabilised; however, there was an overall reluctance to pay for the platform. The findings provide insights into existing barriers and challenges for adoption of ICT for older people living with cognitive impairment.

BACKGROUND: Compromised splanchnic perfusion and the resulting intestinal mucosal injury leads to a decreased mucosal barrier function, which allows translocation of intestinal flora and endotoxemia. The authors evaluated the effects of milrinone on splanchnic oxygenation, systemic inflammation, and the subsequent acute-phase response in patients undergoing coronary artery bypass grafting. METHODS: This open, placebo-controlled randomized clinical study enrolled 22 adult patients in two groups. Before induction of anesthesia, baseline values were obtained and patients were randomized to receive milrinone (30 microg/kg bolus administered progressively in 10 min, followed by a continuous infusion of 0.5 microg x kg(-1) x min(-1)) or saline. The following parameters were determined: hemodynamics; systemic oxygen delivery and uptake; arterial, mixed venous and hepatic venous oxygen saturation; intramucosal pH (pHi); and mixed and hepatic venous plasma concentrations of endotoxin, interleukin 6, serum amyloid A, and C-reactive protein. RESULTS: Milrinone did not prevent gastrointestinal acidosis as measured by pHi, but its perioperative administration resulted in significantly higher pHi levels compared with control. Venous and hepatic venous endotoxin and the interleukin 6 concentration were reduced significantly in the milrinone group. Serum amyloid A values were attenuated in the milrinone group 24 h after surgery. No significant differences could be seen in routinely measured oxygen transport-derived variables. CONCLUSIONS: Perioperative administration of low-dose milrinone may have antiinflammatory properties and may improve splanchnic perfusion in otherwise healthy patients undergoing routine coronary artery bypass grafting. 相似文献

During the recent years, several new Information and Communication Technology solutions have been developed in order to meet the increasing needs of elderly with cognitive impairments and support their autonomous living. Most of these solutions follow a human-centred paradigm that aims to provide users with personalised services according to their needs by also ensuring their safety with mechanisms that can automatically trigger appropriate actions in situations where there may be a risk for an elderly. The present paper presents a hybrid matchmaking approach that uses efficiently both a rule-based and a statistical matchmaker in order to (a) propose ambient assisted living services to the end-users, based on their role, status and context of use and (b) identify and resolve problematic cases by automatically selecting the most proper set of services to be called in a single or combined manner.
This study investigated how culture influences the association between autobiographical memory retrieval and depression. Thirty clinically depressed patients and 30 controls, 15 each from Britain and Taiwan, completed the English and Chinese versions of the Autobiographical Memory Cueing Task (AMT). Overall, the depressed individuals from both cultural groups retrieved significantly fewer specific and more categoric autobiographical memories than their matched, nondepressed controls. Within the control groups, the British participants retrieved significantly more specific autobiographical memories and fewer categoric memories than their Taiwanese counterparts. These results suggest that difficulty in retrieving specific autobiographical memories typical of depression may be a cognitive bias that occurs across cultures. We induced tip-of-the-tongue (TOT) states in elderly participants with probable Alzheimer's disease (AD). We found that they experienced TOTs but, unlike control subjects, were unable to provide any information about the target word for which they were searching. The related words produced by the AD participants were almost all semantically related to the target, with very few phonological relatives. (Adults normally produce more phonological relatives than semantic.) We examine the relationship between the target and non-target words produced in terms of their syntactic category, frequency, and imageability. The results are discussed with regard to their implications for speech production models. We interpret the results in terms of a two-stage interactive account where the retrieval deficit in dementia lies between the semantic and lexical levels. 相似文献
Older people with dementia are a particularly challenging user group to involve in the process of designing interactive systems
that could assist them. It may also be difficult to involve family caregivers of people with dementia, as they are most likely
to be older themselves and uncertain about technology. Paid care staff, whilst younger, may be unclear about the benefits
of technology and lack confidence in their ability to incorporate it into their work. Over the past 7 years, the authors of
this paper have worked closely with people with dementia, their families and professional care staff to develop and evaluate
a multimedia computer system to support communication between people with dementia and caregivers. To achieve this, a number
of user involvement issues were addressed, ranging from legal and ethical considerations of working with people with dementia
to the reluctance of hard-pressed staff to add to their workload for a research project. In addition, developing and conducting
evaluations and eliciting the views of people with dementia who have working memory impairment plus additional cognitive and
social difficulties emerged as a central issue. A variety of approaches were explored within this project, which are described
in this paper, including familiarizing the whole team with the unique difficulties posed by dementia, continuous confirmation
of participants’ consent, and ways to measure enjoyment, engagement, and joint interaction using observation. 相似文献
