Seventy-three spinal cord injured patients with central cord syndrome who had undergone inpatient rehabilitation, were studied retrospectively with regard to their demographic, neurologic and functional characteristics. There were 67 males and six females with a mean age of 53.5 years. Falls was the commonest mechanism of injury (54.8%) followed by motor vehicle accidents. Eleven patients sustained cervical fractures and 41 had radiological evidence of cervical spondylosis. Seventeen patients had sensory impairment and significant spasticity was present in 14 patients. Significant improvements in the admission/discharge ASIA motor scores and Modified Barthel Index (MBI) scores (P < 0.001) were noted after rehabilitation. Ninety-two percent of patients were continent of bladder on discharge compared to 64.4% on admission. Multiple regression analysis revealed three factors associated with a better functional outcome, namely, higher admission MBI scores, absence of spasticity and younger age (P < 0.05).  相似文献

A new mesh generation algorithm called ‘LayTracks’, to automatically generate an all quad mesh that is adapted to the variation of geometric feature size in the domain is described. LayTracks combines the merits of two popular direct techniques for quadrilateral mesh generation—quad meshing by decomposition and advancing front quad meshing. While the MAT has been used for the domain decomposition before, this is the first attempt to use the MAT, for the robust subdivision of a complex domain into a well defined sub‐domain called ‘Tracks’, for terminating the advancing front of the mesh elements without complex interference checks and to use radius function for providing sizing function for adaptive meshing. The process of subdivision of a domain is analogous to, formation of railway tracks by laying rails on the ground. Each rail starts from a node on the boundary and propagates towards the medial axis (MA) and then from the MA towards the boundary. Quadrilateral elements are then obtained by placing nodes on these rails and connecting them inside each track, formed by adjacent rails. The algorithm has been implemented and tested on some typical geometries and the quality of the output mesh obtained are presented. Extension of this technique to all hexahedral meshing is discussed. The objective of this study was to evaluate the antifungal and antioxidant activities of curcumin, ascorbic acid and the mixture of these two compounds. For the antifungal assay, the minimum inhibitory concentrations (MIC) were determined using Candida strains (ATCC and clinical isolates). Curcumin alone inhibited growth of Candida albicans yeast cells, whereas ascorbic acid did not present effects. However, when the mixture of ascorbic acid and curcumin was assayed to determine the association of the two compounds, the curcumin MIC decreased 5- to 10-fold. In the antioxidant assays, the sum of the alone activities of curcumin and ascorbic acid were lower than the activity of the two-compound mixture. This study highlights the importance of the association between two common antioxidants in foods, to improve the antifungal and antioxidant activities of curcumin (in vitro), and can be applied to Candida spp. infection and diseases associated with oxidative stress.  相似文献

Justification of early treatment of nocturnal enuresis is founded in the negative psychological impact on the child. In fact nocturnal enuresis delays early autonomy and socialisation by decreasing in self-esteem and self-confidence. Nocturnal enuresis classification is the preliminary step to correct therapy. Enuresis must be classified as primary (never acquired nocturnal control) or secondary (at least 6 months of dry nights). A child is also classified as having monosymptomatic enuresis if she/he experienced only night wetting and symptomatic enuresis if she/he experienced night wetting associated with diurnal voiding symptoms (urinated > or = 7 times a day, urgency, damp pants, squatting, holding the perineum, sitting on one heel). Monosymptomatic patients must be treated with desmopressin nasal spray at the daily dose of 20 micrograms at bed time. If the reduction of at least the 50% of the basal number of the wet nights is not achieved, the dosage must be increased until 40 micrograms. For patients affected by rhinitis or asthma, desmopressin is now available in tablets. In symptomatic patients desmopressin therapy must be associated to oxybutinin (5 mg x 2). Therapy interruption must be gradual with desmopressin reduction of 10 micrograms every 30 days. In symptomatic patients oxybutinin must be introduced only at bed time. The efficacy of the drugs depends on the therapy length. The highest percentage of success is obtained if the treatment is protracted for at least six months. Antidepressants are also used for nocturnal enuresis especially imipramine. The dosage varies between 0.5-1.5 mg/ kg/daily. As plasmatic levels are achieved only in 30% of treated patients, a 3-5 fold increase in suggested. Nevertheless these levels result in near toxic threshold concentration. Sporadic treatment purposes include amytriptiline, diclofenac sodicum, viloxsazine and methilphenidate if giggle incontinence is present. Non responders may be treated with alarm. If after 16 weeks of treatment no success is obtained alarm use must be interrupted.  相似文献

Products containing silver nanoparticles are entering the market rapidly, but little is known about the potential for inhalation exposure to nanosilver. The objectives of this work were to characterize the emissions of airborne particles from consumer products that claim to contain silver nanoparticles or ions, determine the relationship between emissions and the products' liquid characteristics, and assess the potential for inhalation exposure to silver during product use. Three products were investigated: an antiodor spray for hunters, a surface disinfectant, and a throat spray. Products emitted 0.24-56 ng of silver in aerosols per spray action. The plurality of silver was found in aerosols 1-2.5 μm in diameter for two products. Both the products' liquid characteristics and the bottles' spray mechanisms played roles in determining the size distribution of total aerosols, and the size of silver-containing aerosols emitted by the products was largely independent of the silver size distributions in the liquid phase. Silver was associated with chlorine in most samples. Results demonstrate that the normal use of silver-containing spray products carries the potential for inhalation of silver-containing aerosols. Exposure modeling suggests that up to 70 ng of silver may deposit in the respiratory tract during product use.  相似文献

This paper proposes a framework for generating mesh sizing functions for assembly models. Size control is crucial in obtaining a high-quality mesh with a reduced number of elements. The reduction in the number of elements will decrease computation time and memory use during mesh generation and analysis. The framework consists of a background octree lattice for storing the sizing function, a set of source entities for providing sizing information based on geometric information, and an interpolation module for calculating the sizing on the background octree lattice using the source entities. Source entities are generated by performing a detailed systematic study to identify all the geometric factors of an assembly. Disconnected skeletons are extracted and used as tools to measure 3D proximity and 2D proximity, which are two of the geometric factors. The framework facilitates the generation of a variety of meshes with a low computational cost, to meet industry needs. The framework has been tested on many industrial parts, and sizing control on a few typical assemblies has been presented to demonstrate the effectiveness of the proposed framework.  相似文献

Pre-grouting ahead of tunnels has three main functions: to control water inflow into the tunnel, to limit groundwater drawdown above the tunnel, and to make tunnelling progress more predictable since rock mass quality is effectively improved. It helps to avoid settlement damage caused by consolidation of clay deposits beneath built-up areas, since towns tend to be built where terrain is more flat, due to the clay deposits. There are so many instances of settlement damage that the profession needs to take note of the need for high-pressure pre-grouting, to use micro-cements and micro-silica additives. The use of highpressure injection may cause joint jacking, but this is local in extent when the rapid pressure decay away from an injection hole is understood. This effect is variable and depends on the geometrical parameters of the joints. This pressure-decay advantage must not be violated by maintaining high pressure when grout flow from the injection hole has ceased. The latter can cause damage to the grouting already achieved. Simplified methods of estimating mean hydraulic apertures(e) from Lugeon testing are described, and from more sophisticated three-dimensional(3 D) permeability measurement. The estimation of the larger mean physical joint apertures(E) is based on the joint roughness coefficient(JRC).Comparison is then made with the empirical aperture-particle size criterion E 4 d95, where d95 represents almost the largest cement particle size. Depending on joint set orientations and on the available micro-cements, the decision must be made of which range of pre-injection pressure should be aimed for,using successive reductions of the water-cement ratio w/c. More simple estimation of permeability, also with depth dependence, can be made with the empirical link between a modified rock mass quality Q and permeability, which is termed QH2 O. The value of this parameter can be based on core-logging or intunnel face logging. The 3 D before-and-after-grouting permeability measurements have been used to justify the quantification of rock mass quality Q-parameter improvement, and the consequent increases in expected P-wave velocity and deformation modulus, for application in dam foundation treatment and its monitoring.  相似文献

A Lagrange parameterization of lamination parameters which are used for optimization of variable stiffness laminates is presented. The advantages of the approach are: a) the design variables become independent of the finite element mesh and, b) the smoothness of the solution is inherently guaranteed. Due to independency of design variables of the finite element mesh, the reduction in the number of design variables is drastic, once variable stiffness laminates usually demand a fine mesh. The lamination parameters formulation allows a more precise and concise approach to laminate design, removing difficulties related to fiber direction and stacking sequence, increasing the chances to find a global optimal solution. In this paper, the Lagrange parameterization is used for the maximization of the buckling load of a variable stiffness composite plate.  相似文献
