BACKGROUND: This study examined patients with a first-episode of affective psychosis during acute and compensated states in order to determine whether changes in attentional functioning over time were accompanied by changes in the severity of psychotic or affective symptoms. METHODS: Attentional performance was measured in patients (n = 27) using the degraded-stimulus continuous Performance Test (CPT) and symptoms were assessed at the time of index hospitalization, and 2 months after discharge. A comparison group of normal volunteers (n = 31) also performed the CPT two months apart. RESULTS: Patients performed significantly worse than controls at the initial testing but not at follow-up. The improvement in attentional performance significantly correlated with decreased severity of manic symptoms. CONCLUSIONS: Results suggest attentional dysfunction is a state-dependent characteristic of mania, and may provide an additional measure of clinical improvement following treatment.  相似文献

In Publication 103 (ICRP, 2007), the Commission included a section on the protection of the environment, and indicated that it would be further developing its approach to this difficult subject by way of a set of Reference Animals and Plants (RAPs) as the basis for relating exposure to dose, and dose to radiation effects, for different types of animals and plants. Subsequently, a set of 12 RAPs has been described in some detail (ICRP, 2008), particularly with regard to estimation of the doses received by them, at a whole-body level, in relation to internal and external radionuclide concentrations; and what is known about the effects of radiation on such types of animals and plants. A set of dose conversion factors for all of the RAPs has been derived, and the resultant dose rates can be compared with evaluations of the effects of dose rates using derived consideration reference levels (DCRLs). Each DCRL constitutes a band of dose rates for each RAP within which there is likely to be some chance of the occurrence of deleterious effects. Site-specific data on Representative Organisms (i.e. organisms of specific interest for an assessment) can then be compared with such values and used as a basis for decision making. It is intended that the Commission's approach to protection of the environment be applied to all exposure situations. In some situations, the relevant radionuclide concentrations can be measured directly, but this is not always possible or feasible. In such cases, modelling techniques are used to estimate the radionuclide concentrations. This report is an initial step in addressing the needs of such modelling techniques. After briefly reviewing the basic factors relating to the accumulation of radionuclides by different types of biota, in different habitats, and at different stages in the life cycle, this report focuses on the approaches used to model the transfer of radionuclides through the environment. It concludes that equilibrium concentration ratios (CRs) are most commonly used to model such transfers, and that they currently offer the most comprehensive data coverage. The report also reviews the methods used to derive CRs, and describes a means of summarising statistical information from empirical data sets. Emphasis has been placed on using data from field studies, although some data from laboratory experiments have been included for some RAPs. There are, inevitably, many data gaps for each RAP, and other data have been used to help fill these gaps. CRs specific to each RAP were extracted from a larger database, structured in terms of generic wildlife groups. In cases where data were lacking, values from taxonomically-related organisms were used to derive suitable surrogate values. The full set of rules which have been applied for filling gaps in RAP-specific CRs is described. Statistical summaries of the data sets are provided, and CR values for 39 elements and 12 RAP combinations are given. The data coverage, reliance on derived values, and applicability of the CR approach for each of the RAPs is discussed. Finally, some consideration is given to approaches where RAPs and their life stages could be measured for the elements of interest under more rigorously controlled conditions to help fill the current data gaps.  相似文献

Modeling of the transient thermal state of metallurgical ladels is motivated by the need for estimating the drop in temperature of the liquid metal in the ladle. On-line estimation of the state is required, since the same ladle is used in a number of casting cycles with rapid changes in boundary conditions for the temperature field, and the conditions in the current as well as previous cycles affect the thermal state. Although a large number of methods for the numerical solution of conduction-diffusion partial differential equations have been developed, there are still advantages to keeping thermal field computations at a relatively simple level, in contrast to the situation in the design process of ladles, where two-dimensional modeling may be required. Extensive computations under nonverifiable boundary and initial parameter values are not especially suited for real-time simulation of industrial processes. This article presents a novel approach to the solution of the one-dimensional transient heat conduction problem applied to ladle linings, relying on classical analytical techniques in combination with numerical methods. The performance of the model was validated by a comparison of predictions to thermocouple measurements from the refractory of a steelmaking ladle during a campaign of 26 casting cycles. Reasonable agreement between the measured and simulated variables could be established, which demonstrates the feasibility of the approach.  相似文献

Atmospheric nuclear weapons explosions and large-scale nuclear accidents may contaminate large areas of land with the long-lived radionuclides 137Cs and 90Sr. The mobility and bioavailability of these radionuclides in the environment is dependent primarily on soil characteristics and changes significantly over time after fallout (1-4). Radioisotope concentrations in different rivers and at different times after fallout vary over 2-3 orders of magnitude. Many previous studies have concentrated on the interactions of radiocesium and radiostrontium with various environmental components, but there are currently no operative models fortheirtransport over large spatial areas. We collected time-series measurements of 90Sr and 137Cs in 25 major European and Asian rivers and (using digital data sets with global coverage) determined characteristics of each of the rivers' catchments. This work has established, for the first time, a quantitative link between riverine transport of these radioisotopes and catchment and soil characteristics at a global scale. A generalized predictive model accounting for time changes in river concentrations and variation in catchment characteristics is developed. This can be used to predict the long-term riverine transport of these radiologically important radionuclides following any large-scale nuclear incident in North America, Europe, or (European and Asian) Russia.  相似文献

Sulfur has for long been known as a problematic element for the quality of iron, but along with its accumulation it also causes physical problems in the blast furnace, so it is of central importance to safeguard its removal from the process. Sulfur‐related problems in the blast furnace were studied by applying a balance equation for the element at three industrial blast furnaces, tracking errors in the in‐ and outflows and estimating changes in the amount of accumulated sulfur. A hypothesis on the behaviour of sulfur in the process was proposed and supporting evidence of it was found through an analysis of dynamic phenomena in the hearth of one of the furnaces.  相似文献

BACKGROUND: Nephrolithiasis may be an important consequence of indinavir therapy; however little has been published on the variation in incidence between different populations of patients or the possible mechanisms of calculus formation. OBJECTIVE: To examine variation in the incidence of indinavir-associated nephrolithiasis (IAN) in HIV-positive patients in relation to hemophilia and hepatitis C virus (HCV) infection. METHODS: Clinical data were abstracted retrospectively from the medical records of all adult patients treated with indinavir from September 1995 to September 1997. Occurrence of first IAN, defined as flank pain and hematuria after initiation of therapy, was analyzed in relation to hemophilia status and HCV infection. RESULTS: There were 17 episodes of IAN (22%) among 79 patients treated with indinavir. Of 10 patients with hemophilia, 50% developed IAN as compared with 17% of patients without hemophilia (P = 0.03). Median days to first IAN was 22 (range 7-110 days) for hemophiliacs and 156 (range 5-611 days) for those without hemophilia. Data for HCV status were available for 74 out of 79 patients: 10 out of 27 (37%) patients with HCV developed IAN compared with six out of 42 (14%) without HCV (P = 0.02). CONCLUSION: Overall incidence of IAN was higher than that previously reported and was significantly greater in hemophiliacs than in non-hemophiliacs. HCV may be a contributing factor.  相似文献

Experimental and quantitative image analysis studies are presented to investigate the influence of morphology on the transport of small gaseous penetrants (CO2, O2, N2) in randomly interspersed blends of polystyrene (A) and polybutadiene (B) ranging in volume fractions, фB, from 0.08 to 0.90. The overall transport process, for the blends, appears non-Fickian even though both pure phases A and B exhibit Fickian behaviour. An explanation of this phenomenon, based solely on morphological effects, is presented. A conclusion of the study is that the different methods usually taken as equivalent, to compute the effective diffusion coefficient from sorption data, can give significantly different results. An examination of the morphology is performed by means of digital image analysis on transmission electron microscopy, TEM, micrographs. A significant finding is that, for this system, the maximum intermaterial area density occurs for фB around 0.15.  相似文献

Alpha-tocopherol (vitamin E) may play a role in the treatment of arterial thromboembolic disease, possibly by inhibiting platelet aggregation. Thus far, no clinical evidence exists for this effect. The objective of this study was to assess the effect of alpha-tocopherol supplementation on gingival bleeding either in combination with acetylsalicylic acid (ASA) or without it. This study was an end-point examination of a random sample of male smokers who had participated in a controlled clinical trial, the Alpha-Tocopherol, Beta-Carotene Cancer Prevention Study (ATBC Study) for 5-7 years. The study included 409 men aged 55-74 years of whom 191 received alpha-tocopherol supplementation (50 mg/day); 56 used ASA, 30 received both and 132 received neither. Gingival bleeding was examined by probing with a WHO probe and reported as a percentage of bleeding sites adjusted by the logistic regression model. Gingival bleeding was more common in those who received alpha-tocopherol compared with nonreceivers among subjects with a high prevalence of dental plaque (P < 0.05). ASA alone increased bleeding only slightly. The highest risk of gingival bleeding was among those who took both alpha-tocopherol and ASA (33.4% of probed sites bleeding vs 25.8% among subjects taking neither alpha-tocopherol nor ASA, P < 0.001). In the ATBC Study, more deaths from haemorrhagic stroke and fewer from ischaemic heart disease were observed among those participants who received alpha-tocopherol compared with those who did not. Based on the results of the present study and the ATBC Study, we conclude that alpha-tocopherol supplementation may increase the risk of clinically important bleedings, particularly when combined with ASA.  相似文献

The charts of 311 patients receiving theophylline (T) and 289 patients receiving ipratropium bromide (IB) for COPD were reviewed to determine the total costs and cost-effectiveness of these 2 agents in 3 different health-care settings. A direct cost-accounting method assessed cost, and a Markov decision-analysis model calculated cost-effectiveness. Costs to treat toxic effects were greater for T versus IB. The types and incidences of toxic effects, by drug, were similar among the three centers. Overall costs for T were $121.40 per patient per therapy-month versus $84.56 per patient per therapy-month for IB, as determined by the cost-accounting method. The marginal cost was $366 for T over IB when extrapolated over 1 year using the Markov model. The Markov model also predicted that patients receiving IB had a greater number of complication-free therapy-months (measurement of effectiveness) than patients receiving T. We conclude that treatment with IB was less costly and more cost-effective than T.  相似文献
