STUDY DESIGN: Prospective cohort study. OBJECTIVES: To assess the amount of scar tissue by viewing magnetic resonance images, and to evaluate the correlation between the amount of scar tissue and clinical outcome, surgical technique, and fibrinolytic factors. SUMMARY OF BACKGROUND DATA: The influence of fibrinolytic factors on magnetic resonance images has not been investigated previously. The relation between clinical outcome and findings on magnetic resonance imaging remains uncertain. METHODS: Magnetic resonance imaging at 0.5 Tesla was performed to produce sagittal and axial spin-echo T1-weighted images before and after contrast enhancement on 78 patients 7 years after traditional lumbar discectomy with partial or full laminectomy. Before surgery all patients had been tested for fibrinolytic factors. RESULTS: The overall clinical success rate of the surgery was 73%. No evidence of scar formation was seen in 19 patients, a small amount was seen in 36 patients, a moderate amount in 17 patients, and a large amount was observed in 6 patients. Ten patients who had undergone surgery at two disc levels and 18 who had been treated with full laminectomy exhibited more scar tissue than those patients who had undergone surgery on a single level (P = 0.033) and those who had undergone a partial laminectomy, respectively (P = 0.017). The amount of scar formation also was associated with a poor outcome (P = 0.017) and with low preoperative values of tissue plasminogen activator antigen (P = 0.003) and tissue plasminogen activity (P = 0.048) in samples collected after venous occlusion. The intensity of contrast enhancement, however, was not influenced by these or any other parameters. CONCLUSION: The amount of scar formation after lumbar discectomy seems to be related to the clinical outcome, the size of the surgical exposure, and some fibrinolytic factors.  相似文献

Biomanufacturing processes may be optimized by storing cell culture media at room temperature, but this is currently limited by their instability and change in color upon long-term storage. This study demonstrates that one of the critical contributing factors toward media browning is tryptophan. LC-MS technology was utilized to identify tryptophan degradation products, which are likely formed primarily from oxidation reactions. Several of the identified compounds were shown to contribute significantly to color in solutions but also to exhibit toxicity against CHO cells. A cell-culture-compatible antioxidant, a-ketoglutaric acid, was found to be an efficient cell culture media additive for stabilizing components against degradation, inhibiting the browning of media formulations, and decreasing ammonia production, thus providing a viable method for developing room-temperature stable cell culture media.  相似文献

This study was designed to explore the changes in physico-chemical forms of Pb, Cu, Zn and Sb in a stream draining a contaminated shooting range, located at Steinsjøen in the South-Eastern part of Norway, during a period of 21 days. To obtain information on the element species distribution, an interphased size and charge fractionation system was applied, where membrane filtration (0.45 µm) and ultrafiltration using hollow fibre (nominal cut off 10 kDa) were performed prior to charge fractionation using chromatography (cationic and anionic exchange resins). The results show that Pb mainly was present as particulate and colloidal high molecular mass (HMM) species, Cu as colloidal (HMM) and low molecular mass (LMM) species, while Sb and Zn were mainly present as LMM species. The total element concentrations of Pb, Cu, Zn and Sb were positively correlated to water flow and dissolved organic carbon (DOC), suggesting these are important factors in controlling the run-off of the investigated elements in this catchment. During episodes of higher water flow, the increase in element concentration was mainly in the colloidal fraction. Partial redundancy analysis (pRDA) revealed that variations in pH, HMM organic carbon (HMM OC) and LMM organic carbon (LMM OC) explained 47% of the variation in size distribution of the elements, while variations in precipitation and water flow explained 48% of the variation in the charge distribution of the elements. The variation in concentrations during the period varied by a factor of 4, also stressing the importance of frequent sampling opposed to spot sampling in environmental surveys and risk assessments.  相似文献

Plug-in hybrid electric vehicles (PHEVs) could reduce transportation air emissions and energy use. However, a method is needed for estimating on-road emissions of PHEVs. To develop a framework for quantifying microscale energy use and emissions (EU&E), measurements were conducted on a Toyota Prius retrofitted with a plug-in battery system on eight routes. Measurements were made using the following: (1) a data logger for the hybrid control system; (2) a portable emissions measurement system; and (3) a global positioning system with barometric altimeter. Trends in EU&E are estimated based on vehicle specific power. Energy economy is quantified based on gasoline consumed by the engine and grid energy consumed by the plug-in battery. Emissions from electricity consumption are estimated based on the power generation mix. Fuel use is approximately 30% lower during plug-in battery use. Grid emissions were higher for CO?, NO(x), SO?, and PM compared to tailpipe emissions but lower for CO and hydrocarbons. EU&E depends on engine and plug-in battery operation. The use of two energy sources must be addressed in characterizing fuel economy; overall energy economy is 11% lower if including grid energy use than accounting only for fuel consumption.  相似文献

Four pop-out search experiments investigated whether dimension-based visual attention is top-down modulable. Observers searched for singleton feature targets defined, variably across trials, by a color or an orientation difference to nontargets. Observers were precued to the most probable target-defining dimension (e.g., by the word color) or feature (red) on a given trial. Results revealed expedited reaction times (RTs) for valid-dimension targets relative to neutral-cue conditions, and slowed RTs for invalid-dimension targets. Cue information as to precise target feature yielded some extra effect only for color targets. There is a growing interest in the issues of how to organise healthcare work along individual patient cases rather than along the demarcation lines of healthcare organisations. Health information systems, such as electronic patient records, are seen as important change agents, since they are asserted to help the coordination of care across organisations through fast and accurate exchange of clinical data. The paper explores how a semi-standardised discharge letter is employed to communicate about the patient between two organisational settings, the hospital and the general practitioner. It is shown that the discharge letter plays a double role as informational tool and accounting device. And it is argued that further standardisation of the discharge letter content – in order to facilitate electronic exchange – is likely to strengthen the letters role as a tool for organisational accountability and weaken it as a clinical tool. The paper concludes that this finding adds to the theoretical understanding of how computers support cooperative work, and that understanding how healthcare professionals present themselves as accountable and trustworthy should be of major concern when designing healthcare ICTs.  相似文献

Using a tapered in width square active waveguide and bulk InGaAsP/InP material we demonstrate a polarisation independent amplifier structure operating at 1550 nm with a reduced far-field divergence. Improvement of coupling efficiency enables us to achieve a 25 dB fiber to fiber gain together with 9 dBm fiber saturation output power for 150 mA bias current. In acidified aluminium (Al) rich freshwater positively charged Al-species (Al(i)) are the key toxic components due to the accumulation in fish gills. As a countermeasure, liming is used to increase the pH and reduce the concentration of Al(i)-species; in particular low molecular mass (LMM) Al(i)-species by hydrolyses. However, very toxic high molecular mass (HMM) Al polymers can form in the unstable mixing zone immediately after liming. In the present work gill reactivity of LMM and HMM Al-species was studied under controlled conditions in eight channel-tank mixing zone systems in the field where Atlantic salmon (Salmo salar L.) kept in cages were exposed to defined mixing zone water. Mixing zones were created by continually liming acid river water (pH 5.0-5.7) high in LMM Al(i) to pH 6.0 and 6.4, respectively. Transformation processes affecting the Al-speciation as a function of time after liming were documented by in situ hollow fibre ultrafiltration interfaced with ion chromatography, while the Al accumulation in fish gills was used as bioindicator. For fish exposed to mixing zone water immediately after liming (1 min) the Al accumulation in gills (mug Al g(-1)) was higher (factor of 2) than for fish exposed to acid water prior to liming, due to the formation of gill reactive HMM Al(i)-species. The Al accumulation in gills followed a first order kinetic expression reaching steady-state conditions after 24-h exposures. The deposition rate of Al in gills (mug Al g(-1) h(-1)) correlated with the water concentrations of HMM Al(i) (R(2)=0.80) in the mixing zones, and for LMM Al(i) in the acid water (R(2)=0.92). Due to the transient nature of HMM Al(i) the deposition rate of Al decreased from the point of liming with a factor of 10 downstream the channel-tank system (i.e. 100 min after liming). The concentration of gill accumulated Al was higher (factor of 3) immediately after high level liming (pH 6.4) than following low level liming (pH 6.0). However, high level liming was more efficient in detoxifying Al in downstream waters. Furthermore, the bioavailability of a given LMM Al(i) concentration as well as bioreactivity following liming was dependent on the TOC and silicon concentration in the acid water. Increased concentration of TOC (1.5 to 4.3 mg l(-1)) and silicon (0.3 to 1.0 mg l(-1)) reduced the gill reactivity of Al(i) by approximately 50%.  相似文献

Splicing is an important RNA processing step. Genetic variations can alter the splicing process and thereby contribute to the development of various diseases. Alterations of the splicing pattern can be examined by gene expression analyses, by computational tools for predicting the effects of genetic variants on splicing, and by splicing reporter minigene assays for studying alternative splicing events under defined conditions. The minigene assay is based on transient transfection of cells with a vector containing a genomic region of interest cloned between two constitutive exons. Cloning can be accomplished by the use of restriction enzymes or by site-specific recombination using Gateway cloning. The vectors pDESTsplice and pSpliceExpress represent two minigene systems based on Gateway cloning, which are available through the Addgene plasmid repository. In this review, we describe the features of these two splicing reporter minigene systems. Moreover, we provide an overview of studies in which determinants of alternative splicing were investigated by using pDESTsplice or pSpliceExpress. The studies were reviewed with regard to the investigated splicing regulatory events and the experimental strategy to construct and perform a splicing reporter minigene assay. We further elaborate on how analyses on the regulation of RNA splicing offer promising prospects for gaining important insights into disease mechanisms.  相似文献

The olfactory sensitivity of three groups of Atlantic salmon (Salmo salar L.) toward substances emanating from their own groups was studied. Thresholds were determined by electrophysiological recordings of the induced waves from the medial and lateral part of the olfactory bulb surface. The intestine contained more potent olfactory substances on a gram per liter basis than skin mucus, urine, or amino acids. Chemical fractions were obtained from a parallel study on the nature of the naturally occurring substances by Stabell et al. (1982). A retarded fraction from chromatography on a Sephadex G-25 column contained the most potent material. The most potent fractions of the intestinal content evoked responses mainly in the medial part of the olfactory bulb, whereas the lateral part responded to amino acids. The results suggest that all salmon smolts of the waterways contribute to an odor trail in the coastal currents, thus facilitating the odor-dependent migration of the mature salmon.  相似文献
