OBJECTIVE: To estimate the frequency of perioperative morbidities in patients who underwent anesthesia and a surgical procedure with no preoperative laboratory testing. MATERIAL AND METHODS: We conducted an electronic database search of medical records of 56,119 patients who underwent surgical or diagnostic procedures and anesthesia at Mayo Clinic Rochester in 1994 and found 5,120 who had no laboratory tests done within 90 days before the procedure. From this group, we randomly selected 1,044 patients (87 from each month) to document the absence of preoperative tests, the presence of preexisting disease (by organ system), the type of anesthetic agent, and the outcomes and tests intraoperatively and postoperatively. RESULTS: The 1,044 patients ranged in age from 0 to 95 years (median age, 21). No deaths or major perioperative morbidities occurred (0.0%; exact 95% confidence interval, 0.00 to 0.35%). Although 10 patients underwent blood typing and screening for antibodies immediately preoperatively, no blood transfusions were necessary. Intraoperatively, 17 laboratory tests and 1 electrocardiogram were obtained, and 3 results were abnormal. Postoperatively, 42 blood tests and 2 electrocardiographic procedures were performed. Five of the 42 blood tests showed abnormal results (hemoglobin levels in 3, serum sodium in 1, and arterial blood gases in 1). One electrocardiogram showed normal findings, and the other revealed normal results except for premature ventricular contractions. No laboratory test done intraoperatively or postoperatively was found to change surgical or medical management substantially. One patient who had unanticipated blood loss during an outpatient procedure was admitted to the hospital for observation. CONCLUSION: All 1,044 patients, 97% of whom were relatively healthy, with no recent laboratory testing safely underwent anesthesia and an operation. We conclude that patients who have been assessed by history and physical examination and determined to have no preoperative indication for laboratory tests can safely undergo anesthesia and operation with tests obtained only as indicated intraoperatively and post-operatively. Current anesthetic and medical practices rapidly identify perioperative indications for laboratory evaluation as they arise.  相似文献

A recently built experimental setup was employed for the estimation of the solid–liquid equilibria of alternative refrigerant systems. In this paper two binaries, i.e., carbon dioxide + trifluoromethane (CO2 + R23) and nitrous oxide + trifluoromethane (N2O + R23), were studied down to temperatures of 117 K. In order to check the reliability of the apparatus, the triple points of the pure fluids contained in the mixture were measured, revealing good consistency with the literature. The results obtained for the mixtures were interpreted by means of the Schröder equation.  相似文献

The process of neutralization with NaOH, in the presence of Fe(III) salt, of sulphuric acid battery industry wastewater seems to be more suitable than any other process for Pb removal because at the same time, it allows the exploitation of the scavenger action of Fe(III), which is often present in the same wastewater and precipitates as hydroxides. In order to optimize sludge production, a laboratory research study has been carried out to minimize the quantity of Fe(III) to be added. All aspects concerning the chemistry of wastewater have been taken into consideration step by step and discussed. As a result, a process which requires the addition of Fe(III) so that an Fe/Pb ratio, both expressed as ppm, of the order of 0.5 is achieved. The process has been applied to two artificial solutions, characterized by a content of H2SO4 of 1000 and 5000 ppm, respectively, and by 10 ppm of Pb, as well as to five samples of wastewater. The proposed process takes about 1 h to perform: 30 min for neutralization up to pH 8.5–9.5 and Fe(III) addition and 30 min for correction in the 9–9.5 pH range and for flocculation with the help of a polyelectrolyte. The Pb concentration of the treated effluent is below 0.2 ppm. Conditions for Pb removal using only NaOH or Na2CO3 have also been considered.  相似文献

Solid oxide fuel cell (SOFC) power generators can now be commercialized as heat and power micro-cogenerator. Few well-documented field tests have been conducted to date on these units’ tubular cell architecture, however, and little has been done to derive general rules for a thorough understanding of these units’ operation. The present work focuses on characterizing the hydrogen-powered Acumentrics Gen521 (rated 2.5 kW) under various stable conditions. A test rig was installed at the Dipartimento di Energetica of the Università Politecnica delle Marche (Ancona, Italy) to ascertain the main characteristic curves of the Acumentrics Gen521. A multivariate data analysis was performed on the experimental data collected to establish the operating parameters most influential for the stack voltage (SV) and the DC stack output power generated in different working conditions. Some multi-linear response surfaces are suggested for predicting the SV and the DC power in different operating conditions.  相似文献

In this paper, a novel composite sorbent “lithium chloride in silica gel pores” is proposed for application in solar-powered adsorptive ice makers. A mathematical model was used in order to calculate the performance of an ice-maker using this material as adsorbent and methanol as adsorbate. The results of the model showed that a maximum solar coefficient of performance (COPs) of 0.33 and a maximum daily ice production (DIP) of 20 kg m?2 can be obtained for an ice-maker equipped with a solar collector area of 1.5 m2 and 36 kg of adsorbent material. Such performance are noticeably higher than those obtained using commercial activated carbon, that is the adsorbent mostly proposed till now.  相似文献

A recently built experimental setup for determination of solid–liquid equilibria was slightly modified. The action taken on the existing system focused on correcting the drawbacks of the cooling system. In this version of the setup, a direct liquid nitrogen supply was preferred. By means of the modified apparatus, solid–liquid equilibria of the CO2 + R143a and N2O + R143a binary systems were studied. The triple point of R143a was measured to check the reliability of the modified apparatus, revealing good consistency with the literature. The system’s behavior was measured down to temperatures of 148 K. The results obtained for the mixtures were interpreted by means of the Schröder equation.  相似文献

A process for the sulphate removal from the sea water fed to an evaporation plant by means of a weak anion exchange resins plant has been tested both in laboratory and pilot plant scales. The evaporation plant has been operated for 7500 hours at temperatures up to 145°C and with concentration factors varying between 2.0 and 2.5.
