| 摘 要： | 目的探讨阴道镜联合病理活检对宫颈病变筛查的应用价值。方法以我院2007年1月至2009年12月妇产科行宫颈病理学检查患者614例为研究对象,随机分为阴道镜联合病理活检组(简称联合组)372例和病理活检组(简称活检组)242例,比较2组宫颈病变检出率及联合组不同病理分型宫颈癌的阴道镜影像表现。结果 2组对宫颈糜烂、内膜异位、尖锐湿疣、宫颈息肉等常见宫颈良性病变检出率无显著性差异(P>0.05);联合组宫颈癌检出率为11.98%,显著高于活检组(P<0.01);病理报告正常者33例,占13.64%,显著低于活检组(P<0.05);联合组共检出宫颈癌29例,其中鳞状细胞癌13例,占44.83%,腺癌13例,占44.83%,腺角化癌3例,占10.34%。阴道镜下图像以白色上皮表现最多见,占41.38%,其次依次为血管病变和猪油样改变,分别占34.48%和19.94%,另有2例为正常图像,占6.90%。结论阴道镜检查联合镜下病理活检可有效提高宫颈癌早期检出率,不同病理类型宫颈癌在阴道镜下现象具有多形性,单独应用阴道镜具有局限性,临床确诊仍需病理活检。
