CO2激光治疗耳廓良性肿物151例临床观察

目的:观察CO2激光治疗耳廓良性肿物的治疗效果。方法: 耳廓良性肿物151例165个病灶, 麻醉成功后对疣状凸出于皮肤的病灶行活检, 再用功率3～5 W的CO2激光非接触式照射汽化剩余病灶至完全清除; 对位于皮下的结节则选取适当的切口, 用功率4～6 W的CO2激光非接触式照射切开皮肤, 暴露病灶, 用止血钳将内容物及内壁包膜取出送病检, 再用功率2～4 W的CO2激光非接触式照射修整病灶基底部。在烧灼的过程中应特别注意根据病灶所在位置的耳廓自然形状对病灶基底部进行塑型, 使之与耳廓形状相呼应。结果:本组151例病例165个病灶随访1年。其中优142例, 占86.1%; 良 20例, 占12.1%, 差3例, 占1.8%。复发18例, 占10.9%, 瘢痕5例, 占3.0%。结论: CO2激光治疗耳廓良性肿物, 手术操作简单, 可完整祛除病灶, 并可根据耳廓自然形态对病灶基底部其进行精雕细琢而对病灶以外的正常组织损伤少, 使术后耳廓外观形态良好, 美容效果佳。

Clinical Observation on 151 Cases of Benign Neoplasm on Auricle by CO2 Laser Treatment

Objective:To observe the effective of CO2 laser in treating auricular benign neoplasm.Methods: All the 151 cases with 165 lesions were local anesthesia before the treatment. Biopsy first When the lesion is verrucous ,then vaporization the remain part completely with 3～5 W power of CO2 laser in non-contact irradiation . When it is a subcutaneous nodule,biopsy after incising the cutaneous appropriately,then remove all the contents.It should be attention to plastic the basilar part according to the natural shape of the auricle in progress.Results: all of the 165 lesions were follow up in one year.It shows 86.1%,12.1% and 1.8% in good result,moderate result and poor result respectively.There were 18 cases of recurrence,5 cases of scar accounting for 10.9% and 3.0%.Conclusions:We conclude that it is simple and superior cosmetic result in treating the benign neoplasm on auricle by CO2 laser with low recurrence rates and scars.
