2型糖尿病患者中肾病眼病与糖尿病足的相关性

目的观察2型糖尿病患者视网膜病变、糖尿病肾病与糖尿病足之间的关系,探讨早期发现糖尿病足的方法与临床价值。方法随机选取2006年11月至2007年5月我院收治的2型糖尿病患者(无糖尿病足)45例及2型糖尿病并发糖尿病足患者45例,对上述共90例患者进行眼底检查,同日行尿常规及尿白蛋白/肌酐检查,判断有无视网膜病变及肾脏损害。结果非糖尿病足组占糖尿病视网膜病变6.67%(3/45),而糖尿病足组占糖尿病视网膜病变79.9%(36/45),两组视网膜病变发病率有显著性差异(P<0.01)。非糖尿病足组尿常规示尿蛋白阳性( )者占2.22%(1/45),尿白蛋白/肌酐阳性率为6.67%(3/45),两者之和为8.89%;而糖尿病足组尿常规示尿蛋白阳性( ～ )者占20%(9/45),尿白蛋白/肌酐阳性率为46.7%(21/45),两者之和为66.7%(30/45)。两组尿蛋白阳性率存在显著性差异(P<0.01)。结论糖尿病患者视网膜病变的发病率、糖尿病肾病的发病率与糖尿病足的发病率有相关一致性。临床上可以通过检查病人的眼底、尿常规或尿白蛋白/肌酐,针对结果异常的患者进行进一步检查,来早期发现糖尿病足并防止其迅速发展,以此降低糖尿病病人的致残率。

The Relevance of Nephropathy Retinopathy and Diabotic Foot in T2DM

Objective To early diagnosis of diabetic foot(DF) and clinical,we analyzed the relations among retinal disease,diabetic nephropathy and diabetic foot in diabetic patients.Methods From Nov 2006 to may 2007,90 cases of patients,45 with diabetic foot and 45 without diabetic foot,were tested of urine,protein/creatine of urine and eyeground to diagnosis the existence of retinal disease and malfunction of kidney.Results 6.67%(3/45)patients of non-DF group had retinal diseases against 91.1%(41/45)of DF group.In non-DF group,the positive morbidity is 2.11%(1/45) of protein in urine,and is 6.67%(3/45) of protein/creatine in urine,addition of them is 8.89%.In DF group,the positive morbidity is 20%(9/45),46.7(21/45),66.7%,separately.There were significant differences of positive morbidity between two groups.Conclusion They were positive correlation among retinal disease,diabetic nephropathe and diabetic foot in diabetic patients.It indicated that testing urine routine,protein/creatine of urine and eyeground,were optimal methods in clinical to early diagnasis of diabetic foot,the complication of diabetes,to approach advanced treatment and decrease the morbidity.
