CO2激光治疗耳鼻咽喉部乳头状瘤

目的本文报道CO2激光治疗耳鼻咽喉部乳头状瘤172例,并取得满意的疗效,它是一种简单和有效的方法.方法172例鼻部乳头状瘤45例,耳部乳头状瘤39例,咽部乳头状瘤82例,喉部乳头状瘤6例.治疗局部或全身麻醉后,以连续输出功率为4～20W的CO2激光切割及气化肿瘤,切取获得标本可供病理切片.术后创面有一层凝固膜,约2～3周,创面痂膜自行脱落,周围上皮移行愈合,创面伤疤平整.结果治愈率达100%.讨论CO2激光治疗耳鼻咽喉部乳头状瘤的优点(1)生物组织含70%～90%的水分,激光气化肿瘤,对邻近组织基本不受影响,不妨碍伤口Ⅰ期愈合,(2)激光切割和气化肿瘤后,可最大限度保存原有器官的功能,不造成组织缺损,(3)激光能封闭和凝固肿瘤组织周围的血管,术时出血少,(4)激光是无菌切口,很少引起伤口感染,(5)手术简单,痛苦少.

Treated Otorhinolaryngologicai Papilloma with CO2 laser

Objective:172 cases of otorhinolaryngologicai papillomas treated with CO_2 laser,and obtained satisfied effect.So the method is believed simple and effective.Method:Among 172 cases were 45 cases of nasal papilloma,39 cases of aural papilloma,and 82 cases of pharyngeal papilloma and 6 cases of laryngeal papilloma.For local or general anaesthesia,cut and vaporize tumor with CO_2 laser in 4-20W of continuous output power,cut specimen as the pathologic segment.There is a piece of coagulating membrane after operating,but wound scab will drop by itself around 2-3 weeks.Then surrounding epithelia will migration healing,the wound shows smoothness.Result:The healing rate is near 100%.Discussion:Treated otorhinolaryngulogicai papilloma with CO_2 laser have some ad- vantages as follows:(1)There are 70-90% of water in biologic tissues,laser hardly affect contiguous tissue during vaporizing tumor, and not affect healing of wound,(2)After vaporizing and cutting tumor with laser,it can maximum keep the function of original or- gans,and not form tissue defection,(3)The laser can block and coagulate the vessel surrounding tumor tissue,so less bleeding during operating,(4)The laser incision is sterile,rarely affect infection of wound,(5)The operation is simple,so less painful for patient.
