靶控输注依托咪酯脂肪乳在腹部手术中的应用效果研究

目的 探讨依托咪酯脂肪乳靶控输注全身麻醉复合硬膜外阻滞用于腹部手术的可行性.方法 将2012年9月至2013年4月收治的92例行腹部手术治疗的患者按随机数字表法分为对照组和观察组,每组46例,行硬膜外穿刺麻醉效果满意后应用靶控输注系统,对照组给予丙泊酚麻醉,观察组给予依托咪酯脂肪乳麻醉,对2组患者的麻醉效果进行统计学分析.结果 对照组诱导后、插管后即刻MAP、SpO2、HR值均低于观察组（P〈0.05）;观察组患者睫毛反射消失时间、苏醒时间、恢复时间及并发症发生率与对照组比较差异均无统计学意义（均P〉0.05）.结论 依托咪酯脂肪乳较丙泊酚更有利于维持患者全身麻醉诱导期血流动力学的平稳,有利于老年患者的术后恢复.

Application of Target-Controlled Infusion of Etomidate-Containing Lipid Emulsion in Abdominal Surgery

Objective To investigate the feasibility of general anaesthesia with target-controlled infusion of etomidate-containing lipid emulsion combined with epidural block in abdominal surgery. Methods A retrospective analysis was performed in 92 patients who underwent abdominal sur gery from September 2012 to April 2013. These patients were randomly assigned to receive target controlled infusion of either etomidate-containing lipid emulsion （observation group,n=46） or propofol （control group, n = 46）. Anesthetic efficacy was compared between the two groups. Re sults Compared with observation group, the mean arterial pressure, oxygen saturation and heart rate significantly decreased in control group immediately after intubation. There were no signifi cant differences in extinction time of eyelash reflex, awakening time, recovery time and incidence of complications between the two groups （P〈0.05）. Conclusion Etomidate in lipid emulsion is superior to propofol for the maintenance of stable hemodynamics during induction of general anes- thesia and the postoperative recovery in elderly patients.
