新生儿缺氧缺血性脑病IGF-1测定及其临床意义

摘    要:目的 研究新生儿缺氧缺血性脑病(Hypoxic ischemic encephalopathy,HIE)血清胰岛素样生长因子-1(insulin-like growth factor-1,IGF-1)的水平及其临床意义.方法 采用放射免疫法(Radioimmunoassay,RIA)测定30例正常对照、60例HIE患儿急性期及恢复期IGF-1含量,其中HIE轻、中、重度各20例.结果 新生儿HIE急性期及恢复期的血清IGF-1的含量均显著低于正常对照组(P〈0.01),但恢复期IGF-1含量相对于急性期有明显升高(P〈0.01).急性期HIE轻、中、重度各组间的IGF-1含量差异均有统计学意义(P〈0.01),病情越重,IGF-1含量越低.结论 HIE患儿血清IGF-1含量的降低与HIE的病理生理过程密切相关,可作为病情严重程度的监控指标.并为临床应用IGF-1治疗HIE提供理论依据.

Abstract:Objective To study the serum insulin-like and their clinical significance in newborns with hypoxic Serum IGF-1 levels were detected by radioimmunoassay, growth factor-1 (IGF-1) concentrations ischemic encephalopathy(HIE). Methods in 30 normal controls and 60 newborns with HIE in acute and recovery period. Furthermore,these HIE newborns were divided into three group: mild,moderate and severe,with 20 newborns in each group. Results The levels of IGF-1 in newborns with HIE in acute and recovery period were significantly lower than those in normal eontrols(P〈0.01), but IGF-1 levels in recovery period were significantly higher than those in a- cute period (P〈0.01). There were obvious differences in IGF-1 levels among mild,moderate and severe groups(P〈0.01). The severity of illness increased with the reduction of IGF-1 levels. Con- clusion The decreased IGF-1 levels are correlated with the pathophysiological process of HIE in newborns. Therefore,IGF-1 levels can be used as an index to monitor the severity of illness and may provide a theoretical basis for IGF-1 treatment of HIE.
