肾积水程度与经皮肾镜碎石取石术后失血的相关性

摘    要:目的 探讨经皮肾镜碎石取石术(PCNL)后失血与患肾积水程度之间的相关性.方法 回顾性分析2010年1月至2013年3月应用超声引导经皮肾镜气压弹道联合超声碎石术治疗的116例上尿路结石患者的临床资料,根据肾积水程度将其分为轻度、中度及重度3组,比较各组间手术前后的血红蛋白含量和输血情况.结果 3组患者术后血红蛋白含量均出现了下降,轻、中、重度组分别为(14.6±5.1)、(11.4±4.2)、(14.9±9.0)g·L-1,3组比较差异有统计学意义(P〈0.05);术后轻、中、重度组输血率分别为10.9%、4.9%和13.8%,3组比较差异无统计学意义(P〉0.05).结论 患肾积水程度与PCNL后失血量有关,轻度和重度肾积水术后失血量可能要大于中度肾积水.

Abstract:Objective To explore the correlation between the degree of hydronephrosis and re- nal haemorrhage after percutaneous nephrolithotomy(PCNL) . Methods A retrospective analysis was performed in 116 patients with upper urinary calculi who underwent PCNL from January 2010 to March 2013. According to the degree Qf hydronephrosis,these patients were divided into three groups:mild group, moderate group and severe group. Hemoglobin content and blood trans- fusion were compared among the three groups. Results The mean drop in hemoglobin content was (14. 6±5.1)g L-1 ,(11.4±4.2)g L-1 and (14. 9±9.0)g L-l in mild,moderate and se- vere groups,respectively(P〈0.05). Blood transfusion rate was 10. 9%,4. 9% and 13. 8% in mild,moderate and severe groups, respectively(P〉0.05). Conclusion The degree of hydrone- phrosis is related to renal haemorrhage after PCNL. Mild and severe hydronephrosis may lead to greater blood loss than moderate hydronephrosis.
