微量血胆红素仪在测定不同程度黄疸新生儿血胆红素水平中的应用价值

摘    要:目的 探讨微量血胆红素仪在测定不同程度黄疸新生儿血胆红素水平中的应用价值.方法 选取足月新生儿黄疸患儿289例,按静脉抽血生化分析仪测定的血总胆红素水平分为2组:非重症黄疸组(血总胆红素≤342 mol·L-1)251例、重症黄疸组(血总胆红素>342 mol·L-1)38例,再用微量血总胆红素仪测定其血胆红素水平.比较2种检测方法的血总胆红素测量值的差异及其相关性.结果 对非重症黄疸患儿2种检测方法的血总胆红素测量值比较差异无统计学意义(P>0.05),而对重症黄疸患儿2种检测方法的血胆红素测量值比较差异有统计学意义(P<0.01).对非重症黄疸和重症黄疸患儿的血胆红素测量值,2种检测方法均呈正相关(r=0.926,P<0.01;r=0.624,P<0.01).结论 对非重症黄疸患儿,由于微量血胆红素仪测定操作简单、费用低廉,且对新生儿损伤小,可基本替代静脉抽血生化分析仪测定;而对重症黄疸患儿,微量血胆红素仪测定不宜替代静脉抽血生化分析仪,只能作为参考或动态监测.

Abstract:Objective To investigate the value of trace bilirubin determination in different de- grees of neonatal jaundice. Methods According to venous blood total bilirubin levels measured by automatic biochemical analyzer, 289 full-term newborns with jaundice were divided into two groups:non-severe jaundice group(total bilirubin levels≤342 mol · L-1 ,n=251) and severe jaun- dice group(total bilirubin levels〉342 mol · L-1 ,n=38). Then, blood total bilirubin levels were determined using trace bilirubin analyzer. Results were compared between the detection methods. Results The total bilirubin levels measured by automatic biochemical analyzer were significantly different from those measured by trace bilirubin analyzer in severe jaundice group(P〈0.01) ,but not in non-severe jaundice group(P〈0.05). However,there was a positive correlation between the two detection methods in both non-severe jaundice group (r= 0. 926, P〈0.01) and severe jaundice group (r=0. 624,P〈0.01). Conclusion For non-severe jaundice newborns,trace biliru- bin analyzer may replace automatic biochemical analyzer for the determination of bilirubin levels due to the simple operation, low costs and small damage. For severe jaundice newborns, automatic biochemical analyzer can not be replaced by trace bilirubin analyzer,which is only used as a reference or dynamic monitoring.
