俯卧位直肠充气螺旋CT扫描对直肠癌的诊断价值

目的:评价俯卧位直肠充气CT检查在直肠癌中的诊断价值。方法:对内镜诊断或临床怀疑直肠肿瘤的68例患者进行俯卧位直肠充气后的螺旋CT容积平扫及增强扫描。逐例观察肿瘤的部位、大小,增强特征,生长方式,侵犯层次,管周筋膜情况,管周淋巴结有无肿大及远处有无转移。结果:68例患者均顺利完成俯卧位直肠充气螺旋CT检查。表现为直肠壁局限性增厚25例,直肠壁环形增厚及变窄17例,直肠肿块26例,管周侵犯或远处转移32例。结论:俯卧位直肠充气CT检查操作简单、经济,不仅可以定位,而且还可以清晰的显示病变与周围组织的关系,对直肠肿瘤的诊断有着较好的实用性。

Diagnostic Value of Spiral CT Scan with Prone Position and Rectal Aeration for Rectal Carcinoma

Objective： To evaluate the value of CT scan with prone position and rectal aeration in rectal carcinoma. Methods： 68 patients diagnosed by endoscope or suspicious of having rectal tumor in clinical diagnosis underwent spiral CT volume plain scans and enhancement scanning after prone position and rectal aeration; To observe the content including： the location and size of tumor, the manifestatinon of plain scans and enhancement scanning, the growth pattern of tumor, the encroachment range of tumor, the condition of peri-rectum fascia, the lymphadenectasis in peri-rectum, and the condition of distant metastasis. Results： 68 patients were successfully completed spiral CT scan with Prone position and rectal aeration. Local thickening of the rectal wall were found in 25 cases, ring thickening of the rectal wall in 17 cases, issue mass in 26 eases （including only extrude into the inner of the rectum in 18 cases, extrude into inner and outer of the rectum in 8 cases）, and surrounding invasion or distant metastasis in 32 eases. Concliusion： The method is simple and economical with high accuracy and clinical usefulness.
