高强度聚焦超声（HIFU）治疗儿童实体瘤的麻醉分析

目的：探讨高强度聚焦超声（HIFU）治疗儿童实体瘤麻醉方法及特点。方法：回顾性分析16例HIFU治疗儿童实体瘤的麻醉方法,采用丙泊酚、咪达唑仑、瑞芬太尼和顺式阿曲库胺静脉复合,间断吸入七氟烷,气管内插管控制呼吸,术中持续监测BP、HR、ECG、SpO2、PETCO2及鼻咽温。记录麻醉诱导时、手术开始、术中1h、结束前1h及清醒拔管后的BP、HR及动脉血气。结果：16例患儿血流动力学稳定,各时点血气分析正常。麻醉时间184min至648min（330±42）min,苏醒时间3min至11min（5±3.2）min;无1例并发症发生。结论：静吸复合、气管插管全麻是高强度聚焦超声治疗儿童实体瘤安全有效的麻醉方法。

Analysis on the anesthetic management during curing solid tumors of children with high intensity focused ultrasound

Objective：To study the features and methods of anesthetic management during curing the solid tumors of children with high intensity focused ultrasound（HIFU）.Method： The anesthetic methods of 16 children with solid tumors which were treated by high intensity focused ultrasound（HIFU） was analyzed.The induction of anaesthesia was performed by the administration of intravenous with propofol、midazolam、 remifentanil、cis-atracurium.Discontinuous inhalation of sevoflurane was performed during anesthesia.Endotracheal intubation was performed for mechanical ventilation.BP、HR、ECG、SpO2、PETCO2 and the temperature of nasopharynx during the operation were monitored.The data of BP、HR、arterial blood gas were recorded after induction of anesthesia,at the begining of the operation,1 h after the the begining of the operation,lh before the end of the operation,and after extubation.Results：Hemodynamics of the 16 patients were stable.The electrolyte,blood sugar as well as blood gas were consistently normal.Anesthetic time was between 184min and 648min（330±42min）,the recovery time was 3min～11min（5±3.2） min.There were no complications after the operations.Conclution： Anesthesia that is performed with administration of intravenous combined with tracheal intubation and inhalation anesthesia is a safe and effective anesthetic method for solid tumors of children during curing with HIFU.
