Value analysis of transvaginal color Doppler ultrasound combined with serum markers for ovarian teratoma

Objective:To explore the value of transvaginal color Doppler ultrasound combined with tumor-associated carbohydrate antigen 125(CA125),CA199 and alpha-fetoprotein(AFP)in the diagnosis of ovarian teratoma.Methods:The data of 90 patients with ovarian teratoma who were admitted to the hospital from January 2014 to February 2019 were analyzed retrospectively.All patients were subjected to transvaginal color Doppler ultrasound examination and detection of serum CA125,CA199 and AFP levels.The maturity of ovarian teratoma was determined according to pathological examination results,and the patients were divided into mature group(n=61)and immature group(n=29).The color ultrasound examination results and levels of serum tumor markers in the two groups and in patients with different characteristics of mature ovarian teratoma were compared.The diagnostic value of transvaginal color Doppler ultrasound combined with serum tumor markers in mature ovarian teratoma was analyzed.Results:The proportions of imaging manifestations such as clear morphology,capsule and regular echo,color Doppler ultrasound blood flow indexes such as pulsatility index(PI),blood flow volume and resistance index(RI)in mature group were significantly higher than those in immature group(P<0.05).Serum CA125,CA199 and AFP levels in mature group were significantly lower than those in immature group(P<0.05).The ultrasound findings,color Doppler ultrasound blood flow signal indexes and levels of serum tumor markers were related to tumor diameter,FIGO staging and histological grading in patients with mature ovarian teratoma(P<0.05).The AUC values of color Doppler ultrasound respectively combined with serum CA125,CA199 and AFP for diagnosing mature ovarian teratoma were 0.827,0.806 and 0.893,larger than that of color Doppler ultrasound,CA125,CA199 or AFP diagnosis alone(0.728,0.712,0.743,0.783).Besides,the AUC of color Doppler ultrasound combined with AFP was the largest(0.893).Conclusion:Transvaginal color Doppler ultrasound and serum CA125,CA199 and AFP levels can be used to identify mature and immature ovarian teratoma.Transvaginal color Doppler ultrasound combined with serum CA125,CA199 and AFP can effectively improve the diagnostic value for mature ovarian teratoma.
