肌钙蛋白I在急性心肌梗塞中的诊断价值

摘    要:采用微粒子化学发光技术定量测定急性心肌梗塞患者血清心肌肌钙蛋白(CtnI)。结果表明:AMI患者发病后4.1±2.3h,TNI开始上升,12.1±4.3h达到峰值(水平为1197.7ng/mL),持续10天后逐渐恢复至正常水平(0.015±0.012ng/mL),均小于阳性判定值(0.1ng/mL)。相关分析显示:(1)CtnI对AMI的诊断灵敏度高于肌酸激酶(CK),肌酸激酶同工酶(CK—MK),乳酸脱氢酶(LDH),谷草转氨酶(GOT)及Ⅸ一羟基丁酸脱氢酶(d—HBDH);(2)CtnI灵敏度及特异性明显优于CK—MB活性。是高灵敏度、高度特异的心肌损伤的血清标志物。

Value of Troponin I in Diagnosising Acute Myocardial Infarction

Abstract:Serum cardiac troponin (CTnI) of patients with acute myocardial infarction was tested with microparticle chemiluminescence method. The results showed that the serum cardiac troponin (CmI) began to increase 4.1 ± 2.3 h after the onset of acute myocarclial infarction (AMI), reached peak value (1197.7 ng/mL) at 12.1± 4.3 h for 10 days and then gradually returned to the normal level (0.015 ± 0.012 ng/mL), which were allless than the positive criteria (0.1 ng/mL). It is concluded that the diagnosis sensitivity of CTnI for AMI is higher than that of creatine kinase ( CK), creatine kinase ( CK - MK), lactate dehydrogenase ( LDH), aspartate transam-inase (GOT) and a- hydroxy dehydrogenase (a- HBDH). The sensitivity and specificity of CTnI is remarkably superior to the activity of CK - MB. Therefore, it is a serum marker of myocardial injury with high sensitivity andspecificity.
