Abstract: | Humans, both operators and managers, contribute significantly to functional risk. Engineering risk analysts therefore need to learn and apply human reliability assessment methods. But these contain flaws that must be understood and allowed for, and they don't address the risks posed by senior managers. This article points to the resulting shortcomings in analyses, argues for a new approach, and shows the scope for research into the subject |
