多层螺旋CT增强扫描对闭合性肾脏损伤CT分型诊断的价值观察

摘    要:目的:观察多层螺旋CT增强扫描对闭合性肾脏损伤CT分型诊断的价值。方法:选取我院在2018年11月-2019年6月收治的闭合性肾脏损伤患者30例,对所有患者的MSCT检查资料进行回顾性分析,并给予平扫和增强扫描。结果:患者MSCT表现根据肾脏损伤程度、范围以及部位来分为5型,其中肾包膜下血肿诊断出10例,为Ⅰ型;肾实质局灶性挫伤诊断出9例,为Ⅱ型;肾实质挫裂伤伴肾周围间隙血肿诊断出7例,为Ⅲ型;肾实质粉碎伤伴肾筋膜外血肿诊断出3例,为Ⅳ型;肾蒂撕裂伤诊断出1例,为Ⅴ型。患者肾脏损伤分型等级越高代表附近脏器发生伴发性损伤的几率越大。结论:MSCT诊断闭合性肾脏损伤能明确的显示范围、具有临床应用价值。

Abstract:Objective: To observe the value of multi-slice spiral CT enhanced scanning in the diagnosis of CT classification of closed renal injury. Methods: 30 patients with closed renal injury admitted in our hospital from November 2018 to June 2019 were retrospectively analyzed for MSCT examination and were scanned with plain scan and enhanced scan. Results: The MSCT findings were classified into 5 types according to the degree, scope and location of renal injury, including 10 cases of subcapsular hematoma diagnosed as type Ⅰ;9 cases of focal contusion of renal parenchyma as type Ⅱ;7 cases of diagnosis of renal parenchyma contusion and laceration with perirenal hematoma as type Ⅲ;There were 3 cases of renal parenchyma comminuted injury with extrafascial hematoma as type Ⅳ;1 case of renal pedicle laceration as type V. The higher the grade of renal injury classification, the higher the risk of accompanying injury in nearby organs. Conclusion: MSCT can be used in the diagnosis of closed renal injury.
