非潜入式ITI种植体的临床应用

摘    要:目的评价ITI种植体用于修复牙列缺损的临床效果。方法对36例牙列缺损的患者,植入61枚非潜入式ITI种植体,对部分骨量不足的患者,选用短种植体,或使用上颌窦内提升术。种植体于植入后3~6个月按照常规方法完成上部结构修复。修复后观察12~18个月,按Albreksson标准进行临床效果评估。结果除1枚短植体发生种植体周围炎外,余植体稳定度好;未出现上部基台松动;牙龈组织健康;术后1年内骨吸收小于1 mm;患者主观满意率高,为94.4%(34/36)。种植成功60枚,累积成功率98.5%。结论 ITI种植体用于修复牙列缺损效果较为满意,在延期种植、上颌窦提升术及短种植体等的应用上,初期稳定性可靠,成功率高。

Abstract:Objective To evaluate the clinical effects of ITI implant system in treatment of dentition defects.Methods A total of 61 non-submerged ITI implants were placed into 36 edentulous patients.Osteotome sinus floor elevation or short implants were applied for the treatment of deficient alveolar ridges.Permanent abutments were mounted on the fixture,and crowns were inserted 3 to 6 months after fixture installation.All implants were monitored for 12 to 18 months of clinical follow-up,and the clinical effects were evaluated according to standard of Albreksson.Results Peri-implant inflammation occurred in 1 patient using short implant.No loosening occurred in the other implants and abutments.The marginal bone loss was less than 1 mm 1 year after final restoration.Soft tissue around implants was healthy.Patient satisfaction rate was 94.4%(34/36).Sixty of the 61 implants were considered successfully integrated,resulting in a cumulative success rate of 98.5%.Conclusion ITI implant system in prosthetics of dentition defects can achieve satisfactory tissue integration,with a reliable initial-stage stability and a high success rate in the application of delayed implant,maxillary sinus lifting and short implants.
