64排螺旋CT对烟雾病诊断价值的探讨

摘    要:目的探讨64排螺旋CT在诊断烟雾病中的应用价值。方法收集17例烟雾病患者,所有病例先行CT平扫检查,然后行CT血管造影(CTA)检查,最后利用ADW4.4工作站行容积再现(VR)、最大密度投影(MIP)和多平面重建(MPR)分析。结果 9例表现为单纯脑内出血,其中4例为脑室内出血、3例为蛛网膜下腔出血、2例为背侧丘脑出血;1例同时合并有脑出血和脑缺血灶;7例单纯表现为脑缺血灶和脑萎缩。CTA均清楚显示血管狭窄或闭塞的部位和周围烟雾状血管形成。结论 64排螺旋CT不仅能清楚显示脑实质的病变,如脑出血和脑缺血灶,而且CTA是筛查烟雾病的重要检查方法,在诊断和治疗烟雾病当中具有重要的价值。

Abstract:Objective To discuss the 64-slice CT in the diagnosis of moyamoya disease in the application.Methods To collect a total of 17 cases patients who smoke,In all cases first CT scan examination,and then CTA examination,the final volume rendering using ADW4.4 workstation line(VR),maximum intensity projection(MIP) and multi-planar reconstruction(MPR) analysis.Results 9 cases showed simple intra-cerebral hemorrhage,4 cases of intra-ventricular hemorrhage,3 cases of subarachnoid hemorrhage,two cases of dorsal thalamus hemorrhage;1 case complicated with cerebral hemorrhage and cerebral ischemic lesions;7 cases showed simple ischemic lesions and brain atrophy.CTA clearly indicate the site of vascular stenosis or occlusion of blood vessels in and around the smoke-like shape.Conclusion 64-slice spiral CT can not only clear indication of brain parenchymal lesions,such as cerebral hemorrhage and cerebral ischemic foci,and the CTA is an important examination of moyamoya disease screening method in the diagnosis and treatment of moyamoya disease which has important value.
