超敏C反应蛋白、D-二聚体与急性脑梗死患者颈部血管斑块的关系

摘    要:目的探讨急性脑梗死患者血清超敏C反应蛋白(hs-CRP)及D-二聚体(DD)水平与颈部血管斑块数量的关系。方法以80例48 h以内首发急性脑梗死患者为脑梗死组,选取同期健康体检者60例为对照组。入院后3 d内完善2组颈部血管彩超检查及血清hs-CRP、DD水平的测定。结果脑梗死组hs-CRP及DD水平均较对照组明显升高(P〈0.05);hs-CRP水平与双侧颈动脉不稳定斑块的数目呈正相关(r=0.423,P〈0.01),DD水平与双侧颈动脉不稳定斑块的不稳定性没有明显的相关性。结论血清hs-CRP水平可能反映了急性脑梗死患者颈动脉粥样硬化的不稳定性,而DD水平与颈部斑块的稳定性在该研究中尚不明确,需要进一步大样本研究;血清hs-CRP及DD水平数值的增高,证实其参与了脑梗死急性期炎性反应,且急性脑梗死患者体内存在凝血-纤溶系统异常。

Abstract:Objective To investigate the relationship of the levels of high-sensitivity C-reactive protein(hs-CRP) and D-dimer(DD) and carotid plaques in patients with acute cerebral infarction.Methods Eighty patients with acute ischemic cerebral infarction presenting within 48 hours from onset of symptoms and 60 healthy subjects were enrolled and assigned to infarction group and control group.The carotid arteries were examined with color ultrasound and the levels of hs-CRP and DD were detected within the first 3 days after admission.Results Compared with control group,the levels of hs-CRP and DD obviously increased in infarction group(P0.05).The levels of hs-CRP,but not DD,were positively correlated with the instability of the bilateral carotid artery plaques(r=0.423,P0.01).Conclusion Serum hs-CRP levels may reflect the instability of plaques in carotid arteries in patients with acute cerebral infarction,and the increased levels of hs-CRP and DD may play roles in the acute process of inflammatory response.The coagulation-fibrinolysis system appears to be abnormal in patients with acute cerebral infarction.
