紫癜性肾炎患儿血栓调节蛋白的检测及其临床意义

摘    要:目的探讨血栓调节蛋白(TM)在紫癜性肾炎(Henoch-Schonlein purpura nephritis,HSPN)患儿中的表达及其临床意义。方法采用酶联免疫吸附法(ELISA)测定146例确诊为HSP患儿的血浆TM水平,其中86例经肾活检确诊为HSPN患儿为HSPN组,60例无肾损害患儿为对照组,并与60例健康对照儿童(健康儿童组)进行比较。结果 HSPN组、对照组、健康儿童组患儿血浆TM值分别为(2.36±0.78)、(1.21±0.49)、(0.56±0.63)mg.L-1,HSPN组血浆TM水平高于对照组(P〈0.05),对照组血浆TM水平高于健康儿童组(P〈0.05);HSPN组肾活检病理Ⅰ级、Ⅱ级、Ⅲ级、Ⅳ-Ⅵ级血浆TM值分别为(1.53±0.68)、(1.95±0.67)、(2.76±1.13)、(3.26±0.98)mg.L-1,两两比较差异均有统计学意义(P〈0.05)。结论血浆TM水平在HSPN患儿中高表达,且TM水平越高,肾损害越重。血浆TM水平可以作为监测HSPN的发生及预测其肾脏受损轻重的一个重要指标。

Abstract:Objective To investigate the expression of thrombomodulin(TM) and its significance in children with Henoch-Schonlein purpura nephritis(HSPN).Methods The levels of plasma TM were measured by ELISA in 146 children with HSPN and 60 healthy children.The children with HSPN were divided into two groups:HSPN group(86 children with HSPN diagnosed by renal biopsy) and control group(60 children without renal lesions).Results Plasma TM levels in HSPN group,control group and healthy children group were(2.36±0.78)mg·L-1,(1.21±0.49)mg·L-1 and(0.56±0.63)mg·L-1,respectively,and there were obvious differences among the three groups(P0.05).In the classification of nephritis pathology,gradeⅠ,Ⅱ,Ⅲ and Ⅳ-Ⅵ were(1.53±0.68)mg·L-1,(1.95±0.67)mg·L-1,(2.76±1.13)mg·L-1 and(3.26±0.98)mg·L-1,respectively,and the differences were significant among the four grades(P0.05).Conclusion The levels of plasma TM significantly increase in children with HSPN.Moreover,the higher the plasma TM levels,the more serious the renal lesions.Therefore,the levels of plasma TM may be used as indexes to determine the occurrence of HSPN and the degree of renal lesions.
