颅脑损伤术后顽固性脑膨出的原因和治疗

目的探讨颅脑损伤术后并发顽固性脑膨出的原因及处理方法。方法对36例颅脑损伤术后并发顽固性脑膨出患者的诊疗经过进行回顾性分析。结果 36例患者中,5例因并发严重肺部感染而死亡。按GOS预后评分,持续植物生存状态1例,重度残废4例,中度残废6例,良好恢复20例。结论颅脑损伤术后并发严重脑膨出的原因主要是脑水肿、脑积水和颅内感染。有效运用药物和采取各项措施减轻脑水肿,解除脑积水,预防感染是解决颅脑损伤术后并发顽固性脑膨出的有效方法。

Cause and Treatment of Obstinate Brain Bulging after Operation in Patients with Craniocerebral Injury

Objective To explore the cause and treatment of obstinate brain bulging after operation in patients with craniocerebral injury.Methods The clinical data of 36 patients with craniocerebral injury who suffered from obstinate brain bulging after operation were analyzed retrospectively.Results Five of 36 patients died of severe pulmonary infection.Prognostic scores showed vegetative state in 1 patient,severe disability in 4 patients,moderate disability in 6 patients and good recovery in 20 patients.Conclusion The main reasons for obstinate brain bulging after operation of craniocerebral injury are hydrocephalus,cerebral edema and intracranial infection.Drugs and other measures for reducing hydrocephalus,cerebral edema and intracranial infection are effective in treating obstinate brain bulging.
