乳腺粗针穿刺活检组织条数目对活检准确率的影响

目的探讨乳腺粗针穿刺活检所取组织条数目与穿刺活检准确率之间的关系。方法收集2008年4月至2010年3月间经乳腺钼铑双靶X线摄影检查确定需要进行数字化三维立体定位穿刺活检的病例300例,根据就诊时间顺序按序贯试验分为A、B、C 3组,每组100例。在穿刺时A、B、C组病例每个病灶各取组织条2、3和4条,将穿刺活检病理结果与术后病理对照,统计各组穿刺活检的准确率。结果 3组病例穿刺活检病理诊断与术后病理相符者均为98例,诊断准确率均为98%（P〉0.05）。A、B 2组各有1例穿刺活检病理诊断为乳腺癌可疑,1例乳腺癌误诊为导管上皮内瘤。C组1例B淋巴细胞瘤误诊为慢性炎症,1例浸润性导管癌误诊为乳腺腺病。结论乳腺数字化三维立体定位穿刺活检所取组织条为2、3、4条的诊断准确率无明显差异。

Effect of Stereotactic Core Needle Breast Biopsy Tissue Strip Number on Accuracy of Biopsy

Objective To discuss the relationship between stereotactic core needle breast biopsy tissue strip number and biopsy accuracy.Methods Three hundred cases admitted between April 2008 and March 2010 were examined by Mo/Rh mammography and needed digital 3D localization biopsy.According to admission time,these cases were divided into group A,group B and group C,with 100 cases in each group.Two strips of breast tissue were taken by breast core needle biopsy in group A,3 in group B,and 4 in group C.The results of stereotactic core needle biopsy were compared with postoperative pathological findings,and the accuracy of biopsy was calculated.Results The results of stereotactic core needle biopsy and postoperative pathology were consistent in 98 cases in each group（diagnostic accuracy 98%）（P〉0.05）.In each of group A and group B,1 case was diagnosed as suspected breast cancer and 1 case of breast cancer was misdiagnosed as ductal intraepithelial neoplasia.In group C,1 case of B lymphoma was misdiagnosed as chronic inflammation and 1 case of invasive ductal carcinoma as adenosis of the breast.Conclusion The accuracy of biopsy does not differ among 2,3 and 4 strips of breast tissue taken by digital 3D stereotactic biopsy.
