原发性肝癌三维能量多普勒显像及与血管造影的相关性

摘    要:目的 :评价三维能量多普勒描述原发性肝癌内血管分布的能力。方法 :对 4 3例原发性肝癌患者 4 3个病灶行二维及三维能量多普勒检查 ,其中 14例患者接受了血管造影检查。比较两种能量多普勒超声显示的瘤内多普勒信号丰富程度及血供类型。对 14例接受了二维、三维能量多普勒及血管造影检查的患者 ,定量比较了瘤内的血管分布情况。结果 :1.  4 3个病灶中 ,三维能量多普勒显示瘤内血管分布丰富程度及分布类型与二维能量多普勒均有明显差异 (均 P<0 .0 0 1) ;2 .二维、三维能量多普勒及血管造影显示的血管密度分别是 4 .3% ,4 6 .0 %及 4 8.5 %。三维能量多普勒与血管造影结果高度相关 (r=0 .87,P<0 .0 0 1) ,而二维能量多普勒相关性较低 (r=0 .4 9,P>0 .0 5 )。结论 :三维能量多普勒描述原发性肝癌内血管分布优于二维能量多普勒 ,并与血管造影结果高度相关。

Abstract:Objective:To assess the ability of three-dimensional (3D) power Doppler imaging (PDI) in depiction of vascularity of hepatocellular carcinoma (HCC).Methods:Forty-three patients with HCCs were subjected to two- and three-dimensional PDI examinations,and fourteen of them underwent angiography.The delineated amount of intratumoral Doppler signal and the pattern of blood supply were compared between two methods of PDI in 43 lesions of 43 patients.Intratumoral vascular amount was quantitatively assessed by calculating the vascular density in 14 lesions of patients who received two- and three-dimensional PDI exa-minations as well as angiography.Results:In 43 lesions,the amount of Doppler signal and blood flow pattern depicted by 3D PDI were significantly different from that by 2D PDI (both, P <0.001).The vascular density at 2D PDI,3D PDI and angiography was 4.3%,46.0% and 48.5%,respectively.3D PDI correlated significantly with angiography in quantifying the vascularity ( r =0.87, P <0.001),whereas 2D PDI correlated insignificantly ( r =0.49, P >0.05).Conclusions:3D PDI is superior to 2D PDI in depiction of tumor vascularity in HCC and is a possible alternative imaging means as compared with angiography.
