膀胱全切回肠原位代膀胱改良术的疗效观察

目的评价膀胱全切除回肠原位代膀胱术的临床效果。方法回顾性分析2004年8月至2008年3月我院膀胱肿瘤行膀胱全切原位回肠代膀胱术18例的临床资料。结果所有病人均得到随访。患者除一例外均无瘤存活。手术时间为6～9 h,平均8h。术中出血量为800～1000mL,平均为950mL;代膀胱容量平均300mL,最大尿流率平均15mL/s;剩余尿量平均为30mL,代膀胱排尿状态良好。结论膀胱全切回肠原位代膀胱手术具有膀胱容量大、内压低、正位排尿、可控性能好等优点,是一种较理想的膀胱替代术式,值得临床推广应用。

The Observation of Faunileal or Thotopicneo Bladder

Objective A in to evaluate the efficacy of faunileal or thotopic neo bladder.Metbods 18 cases of bladder tumor treated from Au-gust 2004 to March 2008 with radical cystectomy and ileal orthobpic neobldderwere studied retrospectively.Results All of these pa-tients have been folbwed up for10 to 70 months with a mean of 22 months Operating tine ranged 6 to 9 hours with amean of 8 hours;The blood loss was 800 to 1000mL with a mean of 950 Ml;All patients were survival with no recurrences except case;Urodynamic scarried out after operation showed that the average capacity of the urinary reservoir.The peak urinary flow rate and the residual urine volrune were 300 mL.15 mL/s and 30 mL.Good urinary stream was reported..A little operative complications occurred Conclusion The ileal orthotopic neobladder continent reservoir might be an ideal foam of orthotopic bladder replacement.Characterized by low pressure.Large capacity,or thotop is continent a rination.
