乳腺黏液癌超声表现与相关免疫组织化学指标的研究

目的：探讨乳腺黏液癌超声表现及与免疫组织化学（免疫组化）指标ER、PR、HER-2、P53表达相关性。方法：回顾性分析44例乳腺黏液癌患者（44个肿块）的超声特征及与免疫组化指标相关性。结果：单纯型黏液癌多表现为边界清晰（92.3%）的均质低回声（84.6%）肿块,后方回声增强（76.9%）;混合型黏液癌多表现为伴有分叶征/毛刺征（74.2%）的等回声（58.1%）肿块,伴后方回声增强（32.3%）。黏液癌肿块超声表现为L/T〈1、边缘规整、内呈低回声者ER、PR阳性率高;肿块分叶征/毛刺征者P53阳性率高于肿块规整者;有淋巴结转移者HER-2阳性率高于无淋巴结转移者（P〈0.05）。结论：乳腺黏液癌超声表现对其诊断有重要作用,超声表现与免疫组化指标存在一定相关性。

Study on Ultrasonography Features and Correlation of Immunohistochemistry of Mucinous Carcinoma of the Breast

Objective： Study on the uhrasonography features of mucinous carcinoma of the breast and correlation immunohistochemical indicators as ER, PR, HER2, P53. Methods： Retrospect on forty- four mucinous carcinoma of the breast cases （44 masses）, the imaging characteristics of the uhrasonography and the expression of ER, PR, HER2, P53 were analyzed statistically. Results： The pure type expression as mass with sharp border, homogeneous low level internal echo, behind echo enhance; The mixed type expression as mass with sign of lobulation/spicule, heterogeneous equal internal echo, behind echo enhances. The positive rate of ER, PR expression was found higher in mass LFF〈I, regular edge, low inside echo; The positive rate of HER-2 expression was found higher in lymphatic metastasis than in none; The positive rate of P53 expression was found higher in sign of lobulation / spicule than in regular edge （P 〈0.05）. Conclusion： The uhrasonography features of mucinous carcinoma of the breast has clinical diagnostie value, there are some correlations between ultrasonographic features and immunohistochemical indicators.
